- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA4625-05-Q-0015. PART I ? PROJECT OBJECTIVES ? Remove and install carpet/VCT tile in the 509th Medical Group Clinic and Dental Clinic A1. SCOPE OF WORK: Provide all labor, materials, tools and equipment, necessary for removal and replacement of carpet and installing VCT tile 1. Scope of Work: ? The contractor will furnish minimum design on the project using any cuts, proposed work schedule, method of execution narrative, design calculations, and performances period and critical work path information. ? The contractor staff will be issued hospital I.D. badges that must be worn while on site. ? The contractor staff will be issued base Privately Owned Vehicle (POV) passes for the duration of the work. The contractor will provide this office the employee information required by the Whiteman AFB Security Forces for vehicle passes. ? The parking of contractor POVs will be in the area behind the hospital. ? Whiteman AFB, to ensure access for the fire department, emergency vehicles and daily delivery vehicles will coordinate the use and positioning of the contractors dumpster, delivery trucks, or anything that my impede egress to/from the facility. ? The contractor will notify this office of any time in which the fire alarm system will be interrupted during the duration of the project. Whiteman AFB will have the in-house maintenance staff notify the Fire Department and turn off the system before any welding or burning. The contractor will notify this office once the work is complete, so the alarms can be turned back on. ? The contractor will be responsible for daily clean up. ? All work to be accomplished will meet all state and federal building codes, as this is a hospital, all Life Safety Codes and NFPA codes will also apply. The contractor shall: 1st Floor carpet (Excluding Main hallways and Dental Clinic) Remove Carpet 3850 Square Yards (SY) and dispose Remove Cove Base 10440 Linear Feet (LF) and dispose Scrape and prep floor Remove Furniture and reinstall furniture for each room. Install Carpet 3850 SY Install 10440 LF of Cove Base Provide on-site dumpster. Dental Clinic: Remove 409 SY of carpet and dispose Remove 1100 LF of Cove Base and dispose Scrape and prep floor Install 259 SY of carpet Install 1100 LF of Cove Base Install 60 LF of Transition Strip Replace carpet in Dental Clinic hallways (marked on map) with VCT to match existing in clinic Remove Furniture and re install furniture for each room ( includes all desks, standard file cabinets, book cases) NOTE: All furniture will have all personal items and private files removed by the person who's desk is being removed, all small articles will be moved by user. 2. Location: Bldg. 2032, 509th Medical Group, Whiteman A.F.B. Mo,4. Character of Task. The work to be done consists of furnishing all the necessary labor, transportation, supervision, expenses, and materials for the work described in Paragraph I and elsewhere throughout this Statement of Work. The work shall be executed in the highest quality attainable, in a manner consistent with the codes, criteria, regulations, and guidelines as described in Paragraph 16 of this document, and in strict accordance with the prevailing standards of industry. 5. Cost Estimates. Cost estimates shall be prepared by competent personnel who possess a working knowledge of building system designs and practices, which are able to make professional determinations based on experience, and are capable of applying sound judgment on the particular building system methodologies. 6. Access to Project Site. The Contractor shall coordinate installation passes and parking arrangements with the using agency?s facility manager. 7. Working within the Medical Treatment Facility. Any work that will be done while the building is occupied that will cause interference or disruption of medical facility services shall require a minimum of one weeks notice and must be coordinated with the using agency?s facility manager. a. Storage and Parking. Contractor storage and parking shall be at the job site in an area designated by the using agency?s facility manager. 13. Work Hours, Utility Outages, and Special Conditions. Work is to be done Nights and Weekends. a. Work shall be done in an eight-hour period between 1700 and 0200 hours. Weekend work hours shall be 0730 - 1430 hours Monday through Friday excluding Federal holidays. Work outside of normal working hours, or Federal Holidays, is permitted at the option of the Government, at no additional cost to the Government, with notice to the using agency?s facility manager of at least 48 hours before the start of the scheduled work. Any work done during normal business hours of the clinic and while the area is occupied must be coordinated with Facility Management and phased. Facility Management will work with the contractor to insure contractor access. b. Utility Outages. All utility outages shall be coordinated with the using agency?s facility manager, regardless of whether or not the building, or section of the building, is occupied. The Contractor shall notify the using agency?s facility manager of any utility outages or disruption to medical services at least one week in advance. The Contractor may elect to work outside of normal duty hours at no additional cost to the Government. c. Special Conditions. The Contractor shall provide the work with the least possible inconvenience and interference to the Government. The Contractor shall take all possible precautions to prevent damage to government property. All damaged areas shall be repaired at no additional cost to the Government as directed by the Contracting Officer. The job site shall be maintained to provide a reasonably clean and uncluttered appearance at all times. 14. Safety. The Contractor shall comply with all base fire, safety and confined spaced regulations while completing their work. 15. Governing Standards and Regulations. Analysis, evaluation, and survey work shall be in accordance with and based on the following current publications as well as all others applicable to the safe, efficient, and economical design of medical facility building systems, including all design and construction related Department of Defense or military service branch regulations, handbooks, and operating instructions. a. National Fire Protection Association (NFP A) 101, Life Safety Code b. NFPA 99, Health Care Facilities c. NFPA 99C, Gas and Vacuum Systems d. NFPA 72, National Fire Alarm Code e. NFPA 70, National Electric Code f. NFPA 110, Standard on Emergency and Standby Power Systems g. NFPA 25, Standard for the Inspection, Testing, and Maintenance of Water-Based Fire Protection Systems h. NFPA 13, Standard for the Installation of Sprinkler Systems i. Joint Commission on Accreditation of healthcare Organizations (JCAHO) Environment of Care Standards j. American Society of Heating, Refrigeration, and Air Conditioning Engineers (ASHRAE) Codes and Standards k. Uniform Federal Accessibility Standards 1. National Standard Plumbing Code l. MIL-HDBK-119 1, Medical Military Construction Program Facilities, Design and Construction Criteria. m. All Occupational Health and Safety Agency (OSIIA) Regulations and Standards 16. Contractor Responsibility. None of the above limits the Contractor?s responsibility to perform all work and to furnish all labor, materials, transportation and supervision required to accomplish each task or work. 17. Wage Rates: The prevailing wage rates for subcontracted workers shall be based on the contract's geographical location. A2. SCHEDULE: The anticipated completion of this project is 31 days after receipt of award. A3. DEFINITIONS: 1. Design-Build (DB): as defined by the Department of Veterans Affairs (VA) is the procurement by the Government, under one contract, with one firm or joint venture (JV) for both design and construction services for a specific project. 2. Contracting Officer: The services to be performed under this contract are subject to the general supervision, direction, control and approval of the Contracting Officer. 3. Project Manager: The Contracting Officer's representative responsible for administering contracts under the immediate direction of the Contracting Officer. 4. Contracting Officer?s Technical Representative (COTR): The COTR is the Contracting Officer's authorized representative. The COTR is responsible for protecting the Government's interest in the execution of the contract work. His duties include surveillance of all work to assure compliance with the contract documents, interpretation of the contract documents, approval of changed work, approval of all submittals, samples, shop drawings, etc. The COTR may issue change orders to the Contractor within the limitations set forth in his delegation of authority from the Contracting Officer. 5. Design Build Contract:: This term, if used herein, refers to the Contract(s) to perform the design and construction of the project. 6. Contractor: This term, as used herein, refers to the contractor under this contract or the DB team. - - E N D - - This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-04. The following FAR Clauses and Provisions apply to this acquisition: Provision 52.212-1, Instructions to Offerors-Commercial Items; Provision 52.212-3, Offeror Representations and Certifications- Commercial Items; Clause 52.212-4, Contracts Terms and Conditions- Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items the following subparagraphs apply: (a), (b)(3), (b)(5), (b)(6-11), (c)(1-2), and e; DFARS Provision 252.204-7004, Required Central Contractor Registration (Mar 1998); DFARS Clause 252.232-7009, Payment by Electronic Funds Transfer (CCR)(Jun 1998). Registration is required in the CCR database in order to receive a Government contract. DFARS Clause 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. Offerors shall include a completed copy of FAR provision 52.212-3 and DFARS Provision 252.212-7000 with their quote. The applicable North American Industry Classification System (NAICS) Code is 238330. The Small Business Size Standard is 500 employees. Award will be based on the Best Value to the Government.
