Description
The 375th Contracting Squadron at Scott AFB IL is contemplating an award to conduct an aerobics program at Scott AFB IL for the period of 01 Oct 2005 to 30 Sep 2006. FUNDS ARE NOT PRESENTLY AVAILABLE FOR THIS RFQ. THE GOVERNMENT MAY CANCEL THIS RFQ AT ANY TIME IF FUNDS DO NOT BECOME AVAILABLE. IF FUNDS DO BECOME AVAILABLE THE GOVERNMENT INTENDS TO ISSUE AN ORDER TO THE VENDOR WHO SUPPLIED THE SUCCESSFUL QUOTE. AWARD WILL BE CONTINGENT UPON THE AVAILABILITY OF FISCAL YEAR 2006 FUNDS. This is a commercial purchase using FAR Part 12 and Part 13 procedures. The following information, as required per FAR 12.603(c)(2) applies: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. (ii) The solicitation reference number is F3S3365077A100 it is being issued as a Request for Quotation (RFQ). (iii) The solicitation document, incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-05 and the Defense Federal Acquisition Regulation Supplement DCN 20050726 Edition. (iv) This acquisition is set-aside 100% for small business under North American Industry Classification System (NAICS) code 713940. The small business size standard is 6 million dollars. (v) CLIN(S): 0001, Conduct an aerobics program at Scott AFB IL for the period of 1 Oct 2005 to 30 Sep 2006. Contractor to provide personnel and materials necessary to perform in accordance with (vi) Description of requirements. (vi) Description of requirement: STATEMENT OF WORK GROUP EXERCISE SCOTT AFB IL 01 OCT 05 ? 30 SEP 06 a. The contractor shall provide all personnel, labor and transportation to conduct an aerobics programs at Scott Air Force Base, Illinois. b. Instructors employed by the contractor shall be certified in Cardiopulmonary Resuscitation (CPR) and first aid procedures. Instructors shall also be certified with the American College of Sports Medicine (ACSM), American Council on Exercise (ACE), Aerobic and Fitness Association of America (AFAA) or Coopers Institute. In addition, Instructors must obtain the necessary certifications for specialized programs such as Yoga, Spinning, Pilates, etc. Instructors with expired CPR, first aid or ACSM, ACE, AFAA or Coopers Institute certifications shall not teach any further classes until certification is renewed and proof of renewal has been submitted to Government. c. A minimum one (1) year experience instructing aerobic classes is necessary for the performance of this requirement. Contractor will be required to provide reference(s) upon Government request. d. Contractor shall conduct step, interval (cardio plus strength), Pilates, aerobic dance classes to include high/low impact, kick fit, circuit, P.T. (push-ups, sit-ups and flexibility) and yoga. Contractor must be able to conduct aerobics classes using a head microphone. Ensure all instructors are familiar with sound system and proper operation. e. Cancellation of Classes. The Fitness Center Director may cancel aerobic classes due to mission requirements or insufficient class attendance. The Director will provide the contractor with a minimum twenty for (24) hour notice prior to cancellation. 1. PERFORMANCE a. The contractor shall conduct 19 classes per week. Classes will be conducted Monday through Saturday at the Fitness Center or *Teen Center at the following times: Mon/Wed Saturday 6:00 a.m. ? 7:00 a.m. 9:30 a.m. --- 10:30 a.m. Mon/Wed/Thurs Mon/Wed 9:30 a.m. ? 10:30 a.m. 5:15-6:15 Mon-Fri *Thurs 11:00 a.m. ? 12:00 p.m. 1700-1800 12:10 p.m. ? 1:10 p.m. b. The dates and times for the 19 classes per week can be changed by written notice, from the contracting officer to the contractor. The written notice must be given to the contractor at least 3 calendar days prior to the change. Instructors must arrive at least 10 minutes prior to their scheduled class time. c. Classes will not be conducted on the following federal legal holidays: New Year?s Day 1 January Martin Luther King?s Day Third Monday in January President?s Day Third Monday in February Memorial Day Last Monday in May Independence Day July 4 Labor Day First Monday is September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day Fourth Thursday in November Christmas Day 25 December d. Classes will not be conducted on 375th Wing down days if the Fitness Center is closed for business or base closure because of inclement weather. Fitness Center Director will notify contractor of any closures on down days. e. Classes should include warm up, Aerobic segment with a minimum of 2 heart rate checks, cool down and stretching. f. Instructor must be able to educate class members on safety precautions and effective exercise principles such as properly taking the heart rate. Ensure instructors are aware that they must notify Fitness Center staff of any injuries or accidents associated with the exercise class. Fitness Center staff will notify proper emergency personnel when needed. If staff is not available, the instructor will do so. 2. LOCATION Location of classes is subject to change due to renovation, repair, maintenance or management decisions based on customer needs. The contractor will be notified 24 hours in advance of any change of location. 4. GOVERNMENT FURNISHED PROPERTY The government shall furnish all Steps, Dumbbells and Stereo Systems. 5. CONTRACTOR FURNISHED PROPERTY Contractor shall furnish all materials and equipment, to include exercise balls, wireless microphone system, necessary to conduct aerobics classes in accordance with this statement of work. Contractor shall be responsible for repair and replacement of all contractor furnished equipment. Contractor will be required to provide a routine maintenance plan for all equipment and materials prior to issuance of contract. 6. BACKGROUND CHECKS Contractor Consent to Background Checks The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified by Law Enforcement Agency Data System (LEADS) through the National Crime Information Center. LEADS checks will verify if a person is wanted by local, state, and federal agencies. All contractor and subcontractor `personnel must consent to LEADS background checks. Contractor and subcontractor personnel who do not consent to an LEADS check will be denied access to the installation. Information required to conduct an LEADS check includes: full name, driver?s license number, and/or social security number, date of birth of the person entering the installation, and completion of a background check questionnaire. The contractor shall provide this information using the Scott AFB Form 21, Contractors Consent for Background Check, and shall submit it in conjunction with the contractor?s request for either base or vehicle passes. Completion of a successful LEADS check does not invalidate the requirement for an escort when contractor or subcontractor personnel are working within controlled or restricted areas. Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Persons later found to be undocumented or illegal aliens will be remanded to the proper authorities. The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provisions of this clause. Furthermore, nothing in this clause shall excuse the contractor from proceeding with the contract as required. Badges The contractor is required to provide identification badges for their employees. All contractor personnel shall wear these badges while on duty on the government site. Badges are required to identify the individual, company name, and be clearly and distinctly marked as contractor. Size, color, style, etc. are to be mutually agreed to by contractor and government. The contractor?s identification badge will not be used as an entry requirement for installation entry or into any government designated controlled or restricted area. Contractor Registration of Vehicles on Scott AFB All contractor or contractor employees? vehicles used for the performance of this contract shall comply with all local, state and federal regulations. Additionally any pass shall be surrendered to Security Forces upon demand to positively identify a persons need to be on Scott AFB. Access to Installation during Force Protection Conditions (FPCONs) Access to the installation during various FPCONs. Contractors will be assigned a mission essential designation IAW requirements contained in SAFBI 31-101/Installation Security Instruction. Only the installation commander or the unit commander requesting contract support will assign the mission essential designation. Property Protection Property protection for facility where the contractors? primary work center is located will be the responsibility of the local facility manager and local Government Security Manager, or their duly authorized representative IAW AFI 31-101, The Air Force Installation Security Program and command/local directives. The contractor shall safeguard all government-owned equipment and materials in his/her possession or use. Safeguarding Classified or Unclassified Information The contractor shall meet Air Force standards for storing, processing, and handling classified information and systems. Additionally, all resources (e.g. maps, publication/instructions, photos) provided by the government to assist the contractor in the performance of their contract will be surrendered upon termination of employment or the end of the contract performance period. Non-Disclosure Agreements To safeguard information, the contractor shall enter into non-disclosure agreements with the responsible local security manager. Privacy Act Work on this project requires that personnel have access to Privacy Information. Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations. (vii) Period of Performance ? 1 Oct 2005 to 30 Sep 2006 Location ? ADDENDA TO 52.212-1: The RFQ shall consist of three (3) separate parts; Part I ?Price Quote, Part II ? Technical and Part III ? Past Performance Information. Part I consists of a Pricing Schedule. Part II consists of one (1) copy of the required instructor certifications referenced in paragraph b. of the Statement of Work for each aerobic instructor and Part III consists of three (3) but no more than five (5) of the most relevant contracts performed within the last 3 years. Relevant contracts are those for aerobic instructor services of the same scope, complexity and magnitude of those required under this acquisition. The offeror shall furnish the following information for each contract referenced on the Past Performance Survey attached with this combined synopsis/solicitation: Company/Division Name; Product/Service; Contracting Agency; Contract Number; Contract Dollar Value; Period of Performance; and Name, Address, Fax and Telephone Number of a Point of Contact. The evaluation criteria are: (a) Past Performance (b) Price (c) Technical (pass/fail) BASIS FOR AWARD This is a competitive best value combined synopsis/solicitation in which competing offerors past performance history will be evaluated on a basis significantly more important than price considerations. The evaluation process shall proceed as follows: (A) Initially, offers shall be ranked according to price. An offer?s proposed prices will be determined by multiplying the quantities identified in the Pricing Schedule by the proposed unit price for each Contract Line Item to confirm the extended amount for each. (B) Next, the Government technical evaluation team shall evaluate, on a pass/fail basis, the certificates submitted by the lowest priced offerors (usually, the lowest 3 quotes) against the following evaluation factor: Certifications by American College of Sports Medicine (ACSM), American Council on Exercise (ACE), Aerobic and Fitness Association of America (AFAA) or Coopers Institute and certifications in Cardiopulmonary Resuscitation (CPR) and first aid procedures. Certifications MUST be submitted with quote. These certifications MUST be current. (C) Using the past performance survey, the contracting officer shall review performance information provided on the lowest priced offerors (usually, the lowest 3 quotes) based on (1) the references provided by the offeror and (2) data independently obtained from other Government and commercial sources. The Government reserves the right to seek information on higher priced offerors if none of the lower priced offerors receive an exceptional past performance rating. The purpose of the past performance evaluation is to allow the Government to assess the offeror?s ability to perform the effort described in this RFQ, based on the offeror?s demonstrated present and past performance. The assessment process will result in an overall risk rating of exceptional, very good, satisfactory, neutral, marginal, or unsatisfactory. Offerors with no relevant past or present performance history shall receive the rating ?neutral? meaning the rating is treated neither favorably nor unfavorably. (D) If the lowest priced evaluated offer is judged to have an exceptional performance risk rating, that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. (E) The Government reserves the right to award a contract to other than the lowest priced offer if that offeror is judged to have a performance risk rating of ?very good? or lower. In that event, the contracting officer shall make an integrated assessment best value award decision. (F) The Government intends to award a contract without discussionss with respective offerors. The Government, however, reserves the right to conduct discussions if deemed in its best interest.
