Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation documents, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-05. Solicitation number R3-03-05-52 is issued as a Request for Quotation (RFQ) in accordance with FAR subpart 13.5. This acquisition is a 100% small business set-aside. The Northern American Industry Classification System (NAICS) code is 238910 and the size standard is $12 Million. A firm-fixed price purchase order will be issued under the Test Program for Certain Commercial Items. Period of performance ? 45 Calendar Days. The solicitation is for Santa Fe Demolition Project, with the line items as follows: Item 1, Removal and Temporary Protection of stored materials located inside the building - 1 lump sum; Item 2, Removal and Disposal of Asbestos Containing Materials ? 1 lump sum; Item 3, Removal and Disposal of Demolition Debris ? non-asbestos containing materials ? 1 lump sum. The accepted lump sum quantities will be paid for at the contract unit prices accepted in this contract. Payment shall be full compensation for all labor, supervision, materials, equipment, transportation, supplies and incidentals necessary to provide for full demolition and removal of the Santa Fe Warehouse. Items of work shall include, but not be limited to: Development of Work Plan and a Completion Report; Removal and Temporary Protection of stored materials located inside the building; Removal and Disposal of Asbestos Containing Materials; and Removal and Disposal of Demolition Debris - non- Asbestos Containing Materials. SANTA FE WAREHOUSE DEMOLITION. LOCATION - The Santa Fe Warehouse project is located in Santa Fe, N.M. off of Camino Carlos Rey, on property owned by the Santa Fe National Forest; T17N, R9E, NMPM Section 33, in Santa Fe County. Take Santa Fe County Road 14 (Cerrillos Road) southwest out of downtown Santa Fe, New Mexico, to Camino Carlos Rey. Proceed south on Camino Carlos Rey to the West Capitor Campus entrance of the Library Archives & Records Center (also Santa Fe Public Schools Acadamy Nutrition) located at 1209 Camino Carlos Rey in Santa Fe, NM directly across from Calle De Oriente Norte. Turn on the entrance road and proceed to the tee intersection. Turn right at the tee and approximately one tenth of a mile east of the intersection is a chain link fence and gate. Proceed through the gate, left at the wye through another chain link fence and gate, to the warehouse directly behind the sign: ?Fire Management Prevention Supression? adjacent to the radio tower. The warehouse can be found on 7.5 minute USGS quad ?Santa Fe, NM.? SCOPE OF WORK - The contractor shall submit a work proposal to conduct a full demolition and removal of the warehouse. The building is block and wood construction 25? by 120? x 14? high. The built-up roofing and exterior wall tiles have asbestos containing materials. The warehouse foundation and radio tower pad shall be demolished. All furniture, boxes, equipment and storage materials located inside the building shall be moved onto wooden pallets provided by the contractor to a central location within 50 feet of the warehouse and protected from the elements with heavy plastic sheeting for a period of one week upon completion of the move. The contractor shall address the disposal of the asbestos debris and construction/demolition debris in the work and cost proposal. The COR will sign any required debris manifests. The radio tower attached to the building is included in the demolition and removal. PROJECT WORK ? 1. The Contractor shall notify NESHAP notification at least 10 working days prior to the start of demolition activity. The contractor shall submit a Work Plan (2 copies) including a Site Specific Health and Safety Plan to the COR for approval. This work plan shall include the removal procedure and disposal location for all asbestos containing materials and general (non-ACM) construction and demolition debris. The contractor shall include the qualifications for all subcontractors. 3. The contractor shall remove and dispose of all asbestos containing materials in accordance with the approved Work Plan, meeting OSHA and NESHAP requirements. 4. The contractor shall remove and dispose of all building debris that does not contain asbestos in accordance with the approved Work Plan. The contractor shall prepare a Completion Report (2 copies) that includes signed manifests, and receipts from the final disposal facility. ENVIRONMENTAL PROTECTION - All liquids (oil, antifreeze, etc.) drained from equipment during routine maintenance will be drained into liquid tight containers and recycled or disposed of at an appropriate site off of National Forest. Contaminated soils resulting from petroleum spills will be excavated, placed in appropriate containers and disposed of at an appropriate site off of National Forest land. In the event of any spill or release of petroleum products, the Contractor shall report it immediately to the Contracting Officer's Representative (COR) and the New Mexico State Environment Department, Hazardous Waste Bureau: 1-505-827-9329, Emergencies; 1-866-428-6535, Non-Emergency Voice Mail ? 24 hours; 1-505-428-2500, Non-Emergency Daytime ? Real Live Person. SAFETY - Applicable Federal and State laws and regulations shall be adhered to. Construction warning signs shall be placed according to MUTCD (see Specification 01570). All construction activities shall be clearly flagged and posted to assure the Public's safety. GOVERNMENT-FURNISHED INFORMATION - Asbestos Test Results of roof and exterior wall tile materials shall be provided to the contractor. UTILITIES - Gas, water, electricity and telephone to the building have been shut off or disconnected by the Government. SHOWING OF PROJECT - Arrangements for a showing of the project can be made by contacting Mark Bremer at (505) 438-7651, Monday through Friday, 8:00 am to 4:00 pm. FAR 52.212-2, Evaluation--Commercial Items is applicable. The Government will award the contract to the Contractor whose offer represents the best value to the Forest Service on the basis of (1) Price Reasonableness, and (2) Technical Proposal. Price Reasonableness and Technical Proposal are equally important. The factors are in descending order of importance. PRICE REASONABLENESS ? Criteria (1) Price shall be fair market value for materials, labor, and incidentals as indicated in the specifications necessary to complete this project. TECHNICAL PROPOSAL ? Criteria (1) Past Performance ? (a) Provide the general background, experience and qualifications of the organization. (b) Include a list of similar or related type projects awarded to the firm within the last three (3) years. Include the name of the customer, contact number, dollar amount, time of performance (length of contract, date started and date completed) and the name and telephone number of the contracting officer or procurement official. (c) List any defaults or contract performance problems in the last five (5) years. Criteria (2a) Submit a work plan indicating how the job will be accomplished. The technical approach should contain as much detail as the Contractor considers necessary to fully explain the proposed technical approach or method. The technical proposal should reflect a clear understanding of the nature of the work being performed. Include the proposed schedule showing start/complete dates for individual portions of the work. Include the earliest possible delivery date. The technical proposal must include information on how the project is to be organized, staffed and managed, indicated size of work crew(s) to be assigned to the project. include a list of names and responsibilities of the key personnel assigned to the project. With each name, include the role the person will serve on this project and a short description of recent project work accomplished by the individual. Criteria (2b) Information should be provided which will demonstrate the Contractor?s understanding of the type of work being performed. 1. The technical proposal must include: Quality control and supervision to assure specifications are met and production capability. 2. The technical proposal must also include information regarding financial stability. (i.e., is the company capable of covering all financial obligations for the project, including employee payroll, prior to receipt of contract payment?)
