Description

The Great Lakes Acquisition Center (GLAC) is soliciting firm fixed price quotations to provide a board eligible/certified psychiatrist to perform psychiatric consultation to facilitate patient care, twelve (12) hours weekly, during the hours of 10:00am to 4:30pm for the beneficiaries of the Department of Veterans Affairs Medical Center, Iron Mountain, Michigan. Anticipated period of performance is October 1, 2005 through September 30, 2006 with four additional one-year option years. Anticipated award date is October 1, 2005. Electronic version and hard copy of solicitation will be available on or about August 25, 2005. This procurement is being conducted under the Subpart 13.5 test program in accordance with Parts 12, 13, or 15 of the FAR or some combination thereof. NAICS number is 621112 with a small business size standard of $8.5 million. Electronic solicitation may be obtained FREE at the following URL: http://www.fedbizopps.gov. Requests for hard copy solicitation must be in writing to the GLAC Contact Point and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address.
