- The VA Nebraska-Western Iowa Health Care System intends to convert the following doctors from fee basis status to contract status: Ernest Haffke, James Hoff, Judson Jones, Agapito Lorezo, James Mathisen and Isaac Witkowski. These physicians perform compensation and pension examinations at the VA facility in Omaha, NE. Anyone feeling they can provide these services must be a licensed physician holding their license in any of the 50 states and must perform these duties at the VA facility in Omaha, NE. This service CANNOT be performed remotely. Interested parties can provide their qualifications along with a copy of their license in any on of the following manners: (1) e-mail to Brenda.birks@med.va.gov, (2) fax to the attention of Brenda Birks at (402) 943-5538 or mail literature to Brenda S. Birks/90C, VA Nebraska-Western Iowa Health Care System, Omaha Division, 4101 Woolworth Avenue, Omaha, NE 68105. All responses must be received by September 2, 2005 in order to be considered.
