- (i) This is a combined synopsis/solicitation for the procurement of commercial items prepared in accordance with the format in Subpart 12.6 and Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this no tice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation document and incorporated provisions, clauses and addenda are those in effect through FAC 2005-5 dtd ?26 Aug 2005 and DFAR Supplement, current to DCN 20050726, dtd 26 July 2005. (iii) This Request for Quotes (RFQ) number is W91YOU-05-T-0059 and the place of delivery is Bassett Army Community Hospital, Medical Logistics Warehouse, Bldg 3567, Fort Wainwrigh t, AK 99703. NOTE: Delivery location is different from Contracting Office annotated above. Quote shall include FOB Destination shipping costs to be included in the unit price of the individual items.????(iv) This solicitation is 100% set-aside for Small Businesses. (v) The applicable NAICS is 424210 with a size standard of 100 employees.???? (vi) All prospective quoters must be actively registered in the Central Contractor Registration (CCR) at www.ccr.gov. (vii) This RFQ has four (4) line items (CLINS): CLIN 0001 6 Bottles Drontal Feline50 NTP/N: 175599; CLIN 0002 2 Bottles Drontal Plus 22.7 MG BTP/N 176099; CLIN 0003 Drontal Plus 136 MG30/BT P/N 8724639; and CLIN 0004 4 Bottles, Drontal Plus 68MG, BTP/N 177099. ALL CLINS ARE BRAND NAME ONLY AND THE MANU FACTURER IS BAYER HEALTHCARE. (viii) Delivery Information: REQUIRED DELIVERY DATE MUST BE NO LATER THAN 22 SEPT 2005.
