Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number N68836-05-T-0180 applies and is issued as a Request for Quotation. Only one responsible source and no other supplier or services will satisfy agency requirements. A justification for other than full and open competition has been prepared in support of this requirement. This announcement constitutes the request for the required service under RFQ N68836-04-T-0180. The North American Industry Classification System (NAICS) Code is 611699 and the Small Business Size Standard is number of dollars $8.5M. This procurement will be awarded using the Simplified Acquisition Procedures Test Program outlined in FAR part 13.5. Numbered Note 22 applies. CLIN 0001 is for 1 ea Instructor, Certified by the Texas Department of Health, to teach the EMT course to 30 students at the Fire and Emergency Services, Naval Air Station, Corpus Christi, TX. (See attachment #1, Statement of Work), (Wage Determination #1994-2507, Rev # 26, Date of Last Revision 06/01/2005) will be incorporated in the award.) STATEMENT OF WORK EMERGENCY MEDICAL TECHNICICAN CLASS PERIOD OF PERFORMANCE: SEPTEMBER 19, 2005 THRU NOVEMBER 10, 2005 HOURLY SCHEDULE OF CLASSES 0800 TO 1700 MONDAY THRU FRIDAY The course must be approve by the Texas Department of Health. The instructor must provide a certificate that is issued by Texas Department of Health Coordinator. A copy of the certificate will be submitted with the quote by the prospective instructor. The Navy will not pay for lodging, car rental, meals or transportation. The Contractor will provide the following: 1. One instructor per site. 2. Supplement any equipment the Navy does not have. 3. Furnish all presentations and classroom materials. 4. Supply all written testing materials 5. Supply a computer program where all students can pick and schedule their EMS and Hospital rotations. The Navy will provide the following: 1. Furnish all skills equipment. 2. Furnish each student with a book. 3. Furnish any copies of handouts and skills sheets. 4. Furnish classroom and projection equipment. 5. Furnish at least one (1) instructor to help with skills each day. 6. Furnish a computer location for each student to be able to sign up for clinical time and to report all clinical patient contacts. Navy understands students will be held to a standard, a grade of 80 for classroom work and a grade of 70 on the Final Exam. Students will be allowed to take one (1) Retest.
