Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-05-T-0267. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. The Dwight David Eisenhower Army Medical Center (DDEAMC), Fort Gordon, Georgia, 30905, has a requirement for Scanning Electron Microscopy (SEM) Trained Technician with extensive experience in sample preparation and operation of the JEOL scanning microscope Model JSM 6400 V. The contractor shall provide electron microscopy services in accordance with JCOHO, OSHA, Army DDEAMC, and Department of Clinical Investigation directives and policies. The Department Assistant Chief or a representative will supervise all activities. The standard of performance shall be to exercise the degree of care, skill, and education expected of a senior medical technologist in the profession. Contractor shall be responsible for training in house staff while working under this contract. The Con tractor will work on site at the Department of Clinical Investigation (DCI), Bldg 38705 at Eisenhower Army Medical Center, Fort Gordon, Georgia 30905, for an estimated amount of 120 hours per year, during which time the contractor will also be training one or more DCI employees. Services will be performed on as needed basis, between the hours of 0700  2200 hours (any 4 hour period within this time range) Monday through Friday excluding federal holidays. The period of performance for this service is for 0 1 October 2005 through 30 September 2006 plus 4 one-year option periods. The contractor shall provide on-site services on a case by case as needed basis not to exceed 120 per year. The requirement is f irm fixed price for the procurement of this service with a place of performance at the Dwight David Eisenhower Army Medical Center, Fort Gordon, Georgia, 30905. The North American Industry Classification System Code (NAICS) for this procurement is 81121 9. Proposed rates shall be in accordance with the Service Contract Act, Wage Determination No: 94:2135 REV 27 Area: Augusta, GA. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial, apply to this acquisition. FAR 52.212-2  Evaluation - The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantages to the Government, price and other factors considered. Evaluation factors of (1) Technic al Capability and Qualifications  Offers will be considered only from contractors who are licensed to perform services, documentation is required. (2) Past Performance  The contractor should describe the past experience in providing the services specifi ed in this solicitation within the past three (3) years. No more than three (3) references from all firms for whom you have been providing similar service within the last three years are to be submitted with your. (4) Price (Total price for Base Peri od including Option Year. Past Performance, Technical Capability, Qualifications, an d Availability when combined, are of equal importance when compared to price. The Government reserves the right to determine what constitutes approximately equal in technical capability and past performance. The Government reserves the right to evaluate offerors prices on the basis of price realism and will consider the offerors price as an indication that they understand the scope of the Governments requirement.
