Description
Contractor to provide Speech Language Pathologist services at the VA Medical Center, 4815 N. Assembly St, Spokane WA. 99205 Pathologist shall be certified through American Speech and Hearing Association (ASHA) with minimum one (1) year diagnostic and clinical experience in assessment and treatment of speech and language disorders and dysphagia in acute hospital setting. Contractor shall provide Speech Language Pathologist to the VA Medical Center within two (2) hours of notification from VA. If contractor cannot provide service within 2 hours, contractor shall notify VA immediately. Speech language evaluations shall be performed within two (2) working days after notification. Patients will be seen at least three (3) days per week, or as indicated on a routine schedule posted in the patients room, or as required by the VA. If for any reason the therapist is late or absent, notification must be given. No Weekends, Holidays or after-hours call. The Speech Language Pathologist shall: a. Have an unrestricted license in a US state, territory or US commonwealth; b. Speak English proficiently; c. Provide evidence of demonstrated current clinical competence to perform appropriate services as described herein; d. Be a US citizen; f. Provide evidence of current professional liability insurance; g. Provide vidence of current privileges (if applicable); h. Provide names of most current employer(s) and two peer reference contacts; i. Current copy of BCLS certification; j. Be a team player; k. Have experience in radiographic studies. l. Be computer literate, as VA Spokane Medical Center utilizes a computerized patient medical record; m. Provide evidence that they have a minimum of one (1) year diagnostic and clinical experience in assessment and treatment of speech and language disorders and dysphagia in acute hospital setting. NAICS Code 621340/$6.0 million applies. The solicitation will be made available after August 29, 2005. The due date for this solicitation is September 22, 2005, by 2:00 p.m. pacific time.
