- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Far Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. Requirement is 100% set-aside for small business concerns only. The NAICS for this combined synopsis/solicitation is 238330, size standard is $12M. RFQ number FA4801-05-T-0517 is issued as a request for quotation. This document incorporates provisions and clauses that are in effect through Federal acquisition circular FAC 23001-24, effective 19 Jul 2004. The Statement of Work below describes the government requirement. The government intends to award one lump sum Contract Line Item Number (CLIN) for the following: Provide all labor, materials, tools, equipment and transportation to install government provided new carpet in Building 29, Holloman AFB. All quotes must be e-mailed to joyce.frost@holloman.af.mil or faxed to (505) 572-2107 with attention to Joyce Frost. Quotes are required to be received no later than 12:00 MST, Wednesday, 14 September 2005. All quotes shall be marked with Request for Quotation Number, Date, and Time. Site Visit time and Statement of Work with Specifications follows contract clauses. The provision at FAR 52.212-1 Instructions to Offerors-Commercial Items, applies to this acquisition. FAR 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government. Award will be made based on price and past performance equally. Offerors must include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items, with quote. The clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. 52.212-4 addendum to are included. The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. Specifically, the following clauses cited are applicable to this solicitation; DFARS 252.225-7000 Buy American Act--Balance of Payments Program Certificate; DFARS 252.225-7001 Buy American Act and Balance of Payment Program; DFARS 252.225-7002 Qualifying Country Sources as Subcontractors; DFARS 252.232-7003 Electronic Submission of Payment Requests. All Offerors are encouraged to attend a SITE VISIT on September 8, 2005 at 9:30 AM. All interested parties shall contact Ms Frost or TSgt Buehler with the names of all attending the site visit NLT noon on September 7 for directions to Holloman AFB. All contractors will meet at the Main Gate of Holloman AFB and proceed with Contracting Officer to the site. You will need to arrive no later than 8:30 AM to clear security. Security clearance usually requires a minimum of 30 minutes to 1 hour depending on demand. A valid pictured ID is required for every person entering the base. Current Registration and proof of Insurance is required for every vehicle driven on to the base. The State of New Mexico assesses a Gross Receipts Tax for firms conducting business operation within the State without regard to the location of entity. You are advised that any proposals/bids received will be presumed to include this cost of business and any request for adjustment or proposed/bid prices as a mistake in proposal/bid either before or after award of a contract will not be considered. The tax rate for Holloman Air Force Base [Otero County (Remainder of County)] is listed and available on the Internet at http://www.state.nm.us/tax/trd_pubs.htm Statement of Work 1. Contractor shall pick up and deliver new Government provided carpet stored in Civil Engineering warehouse to Building 29. Carpet manager shall issue carpet to contractor. 2. Contractor shall remove old carpet, cushion and cove base and install new government provided carpet and contractor provided cushion and cove base in the following areas: Military Justice Office, Legal Office, NAF Office, Legal Library, Chief Enlisted Advisor Office, SVS Office, and Court Room. Floor plans will be submitted electronically upon request and at site visit. 3. Contractor shall remove old carpet, cushion and cove base and install new government provided carpet and contractor provided cushion and cove base in the following second floor office areas: hallway, Staff Judge Advocate Claims Office, Finance Office, Public Affairs Office and sections of the MSG Office. Floor plans will be submitted electronically upon request and at site visit. 4. Contractor shall remove 6-inch carpet base and replace with government provided carpet in the courtroom. This area is approximately 100 linear feet. 5. Government provided carpet shall be Patchcraft Cadence; color is Drumbeat. 6. Contractor shall provide and install 4-inch vinyl cove base in the areas that have existing cove base. Some areas that have wooden baseboards will remain. Manufacturer: Johnsonite. Product: 48 gray (120 foot coils). 7. Contractor shall provide and install T cap molding at all transitions. Manufacturer: Johnsonite. Product: 48 gray. 8. Contractor shall provide and install carpet cushion. Manufacturer: Foamex. Style: 31-11 CX2580. Dimensions: ? in thick x 54 inches wide x 60-foot long, high-density rubber, 80 ounce, 15 square yards per roll. 9. Contractor shall not be responsible for the removal and reinstallation of furniture, but must coordinate with the facility manager of Building 29 to develop a schedule/plan for the installation process. 10. Contractor shall coordinate the carpet removal and installation with the Facility Manager in all phases of this carpet removal and installation process to ensure an efficient project. 11. All work must be performed Monday ? Friday, 0700 ? 1600, unless otherwise coordinated the Contracting Officer, Project Manager and Facility Manager. 12. Contractor shall submit to the Contracting Office a list of all personnel and vehicles required for the project within five days prior to start of project. 13. Government does not provide secured storage. Contractor is responsible for deliveries, proper cover and storage of all materials, tools and equipment. Materials left out in the open without proper cover from the elements are subject to be rejected and shall become the contractor?s responsibility. 14. Contractor is responsible for instructing his employees of all Environment and Hazardous Material Safety Data Sheets (MSDS) Reference AFI 91-301 and OSHA requirements. 15. Contractor shall be knowledgeable of and must comply will all applicable Federal, State and local laws, regulations and requirements regarding environmental protection. NOTES: 1. All carpet installations shall be the double stick method. 2. Total ESTIMATED carpet and cushion removal and installation is 2,700 square yard. 3. Total ESTIMATED cove base/transition material removal and installion is 6,100 linear feet. 4. The Government provided measurements are ESTIMATES and are provided for informational purposes only. The Government assumes no liability related to their accuracy. Offerors shall obtain their own measurements (which can be accomplished at the site visit) and price their offers accordingly. 5. Contractor shall provide seam sealer on all seams. 6. Contractor shall install carpet in accordance with manufacturer?s installation guidelines and/or recommendations. 7. Contractor shall verify removal and installation quantities of warehouse carpet. 8. Contractor shall dispose of all old carpet and installation debris off of Holloman AFB and in accordance with applicable laws. (END OF STATEMENT OF WORK)
