- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Requisition number FB55GASCYLINDER is issued as a Request for Quotation (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-04. This acquisition is unrestricted. The contractor shall provide all materials, transport, etc., to supply the following: Non-personal Services - The contractor/vendor shall provide the service/refill of the following US specification cylinders: Aviators Oxygen per MIL-PRF-27210 9.0 x 51.0 247 CF Welding Oxygen per BB-O-925 9.0 x 51.0 247 CF Nitrogen per A-A-59503 9.0 x 51.0 226 CF Type I (99.99%) Carbon Dioxide per BB-C-101 9.0 x 51.0 50 LB Argon per MIL-A-18455 9.0 x 51.0 246 CF Type I (99.997%) Use, handling, and maintenance of all US specification gas cylinders will be complied with Technical Orders 42B5-1-2 TECHNICAL MANUAL GAS CYLINDERS (STORAGE TYPE) date February 2004. Available upon request. General Labor, tools, materials (replace missing cylinder cap) and equipment to repair cylinders (leaky valves) and put all back into a serviceable condition and perform sandblasting on deteriorated cylinders needing paint or cosmetic works. Leaky Cylinders - Drivers must have numbered QA tags available upon delivery. Cylinders will be leak tested by the customer during the delivery process. If cylinders are found to be leaking, a QA tag will be placed on the gas bottle to identify the problem. The gas bottle or bottles will then be taken away by the delivery driver representing the contractor. Please provide areas on both delivery notes and invoices where leaking cylinders and faulty valves can be annotated and the QA tag details be entered. - Return Cylinders that are marked or stamped ?RAFL? back to RAF Lakenheath. - Return Cylinders that are marked or stamped ?RAFM? back to RAF Mildenhall Delivery of cylinders: - Pick-up and delivery to RAF Lakenheath and RAF Mildenhall is to be treated completely separate. - E-mail will be the primary means of communication with the vendor to place orders and coordinate pick-up and delivery times. Hazmart personnel will let the vendor know how many cylinders require servicing. The vendor will reply to the e-mails within 24 hours to arrange for pick-up and/or delivery date/time. Phone numbers will be provided for both RAF Lakenheath and RAF Mildenhall to call upon arriving at the main gate for an escort. - Deliveries will be after 0800 and before 1400 hrs. - Deliveries on Friday should be no later than 1200 hours. - No deliveries must be made on U.S Holidays, Bank Holidays, and weekend. - All delivery/transportation charges must be included with the gas price. - There will be no more than 3-day turnaround for the return of cylinders. - Ensure that all drivers have an up to date ADR License and the vehicle is correctly marked. - Delivery drivers will assist Hazmart personnel in loading/unloading of cylinders for pickup and delivery. Hydrostatic Testing: - Delivery/return of hydrostatic testing will be the same procedures used for filled - Provide accurate information on date and time of pick up for cylinders due out for Hydrostatic Testing, this to be not less than two days prior to pick-up, this to allow the unit time to palletize the cylinders. - Ensure the delivery driver has a point of contact number and name to call on base. - Bake gas cylinders as part of the hydrostatic testing procedure as required. - Provide estimated date and time of return. Sandblasting: - Ensure all written markings on cylinder read (gas type with the appropriate white or black stripes and RAFL or RAFM re-stenciled on). Point of Contact (POC) at Vendor: - Request that there be a primary and an alternate POC at the vendor to deal with cylinder requirements. Hazmart Pharmacy personnel will e-mail the gas requirements to the vendor?s point of contacts. A pick-up/delivery date will be coordinated at this time. When the primary is unavailable the alternate will step in to assist us when needed. Each offer submitted will be evaluated separately. Far provision 52.212-2, Evaluation ? Commercial Items, applies to this RFQ with the following addenda: Paragraph (a) is filled in with (1) Technical Specifications (2) price, (Technical specifications and price, when combined, are equal). Offerors shall include with the submission of their offer a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications?Commercial Items. The clause at FAR 52.212-4, Contract Terms and Conditions- Commercial Items, without addenda and 52.212-5 Contract Terms and Conditions Required to Implement Statues of Executive Orders ?Commercial Items, without addenda, applies to this acquisition (including: 52.222-21, 52.222-26,52.222-35, 52.222-36, 52.222-37 and 52.232-33).
