- 541-145-05 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Award of the contract may be made without discussions, request that most favorable pricing be offered. The solicitation number is 541-145-05 and is issued as a Request For Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01- 9. This is a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 561621 with a size standard of $6 million. Performance period shall occur during months of January, February and March. The contract duration shall be a base period plus two option years. The initial performance period shall commence from the date of award thru September 30,2006. The performance period shall be as follows: Base Year: Award through September 30, 2006. Option Year 1: October 1, 2006 thru September 30, 2007. Option Year 2: October 1, 2007 thru September 30, 2008. Option Year 3: October 1, 2008 thru September 30, 2009. Option Year 4: October 1, 2009 thru September 30, 2010. Louis Stokes Cleveland Veterans Affairs Medical Center (LSCVAMC), Cleveland, Ohio has a requirement for Testing and Preventive Maintenance for the Government Owned Circuit Breakers located at Wade Park, 10701 East Blvd., Cleveland, OH 44106 and Brecksville, 10000 Brecksville Rd., Brecksville, OH 44141. Prospective offerors are required to make a site visit. Offeror is responsible to ensure that they are totally familiar with the physical location and inspection of the Government Owned Circuit Breakers and required work. To schedule site visits contact Wayne Berry at 440-717-2839. Offeror shall provide all labor, supervision, parts, materials, and equipment necessary to test and perform preventive maintenance on the Government Owned Circuit Breakers. The Government Circuit Breakers are identified on Addendum A. Addendum A shall be made available to offerors that schedule site visits. All scheduled Preventive Maintenance shall be performed after 4:30 PM and before 8:00 AM (non-peak hours) during the months of January, February and March. Offeror is to provide the VA with a plan on how it intends to perform the work. This plan shall be approved, by the VA Contracting Officers Technical Representative (COTR) prior to beginning work. Offeror is required to coordinate with the VA any work at least 14 days in advance. The date of actual performance shall be approved by the COTR. 541-145-05 TESTING/PREVENTIVE MAINTENANCE: Offeror shall provide a Preventive Maintenance plan for the VA's approval. The plan shall be consistent with current NFPA/ANSI/ASTM etc. standards and other applicable codes for Circuit Breakers in a hospital setting. DOCUMENTATION OF TESTING/PREVENTIVE MAINTENANCE: Offeror shall provide to the VA written evidence of test completion, to include recommendations of opportunities for improvement or identification of concerns. All documentation, noting services performed and completed shall be provided to VA upon departure for the VA facility presented shall conform to the NFPA/ANSI/ASTM etc. standards. All work is to be performed after normal working hours. As the offeror's employees will be accompanied by V A personnel, the offeror shall be advised regarding the actual gathering location to meet with the VA representatives prior to the performance of scheduled work. Normal working (peak) hours at the VA Medical Center are from 8:00 AM thru 4:30 PM, Monday through Friday, excluding Federal Holidays. There are ten (10) holidays observed by the Federal Government. They are: New Year's Day, Dr. Martin Luther King Jr. Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas and any other day specifically declared by the President of the United States to be a Federal Holiday. Offeror employees when on VA property shall have visible identification with to include a picture, legible typed name and company sign, symbol or logo. Offeror employees shall have a jacket, shirt, or other visible garment (more than a hat/cap) that identifies them as an employee. Invoices shall be submitted after services have been performed and documentation has been received by the COTR. Invoices for services rendered shall be sent to Chief, Fiscal Service (04), 10000 Brecksville Road, Brecksville, OH 44141 and certified by the COTR. Offeror Qualifications: All work shall be performed by factory trained technicians experienced and qualified to work with the type and size of Circuit Breakers listed in Attachment A. All work shall be performed in accordance with the laws, regulations and codes in effect at this time and the Original Equipment Manufacturers specifications. To be qualified to perform and certify this work an offeror must demonstrate that it's employee(s) have the appropriate certifications, licenses or other credentials. The offeror's primary place of business shall be within 60 miles of both Wade Park and the Brecksville facilities. EVALUATION CRITERIA: Technical Proposal: Offerors shall provide in its Technical Proposal it's approach in managing systems of the same or greater complexity than that of the VA. Offeror shall include any certification or licensure for performance of this type of work and the names and individual licenses for the service technicians that will perform the work for the VA. Evidence of this service performed in a hospital setting will be viewed most favorably; Past Performance: Previous work with VA Hospitals or non-Government hospitals for the same or more complex work. Hospital/Medical center service shall be viewed most favorably. Price Proposal: Priced per CLIN per contract year. The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition with the following changes, items (d), (e) and (h) of the standard clause is deleted. The provision at FAR 52.212-3, Offeror Representations & Certifications - Commercial Items is included and the offeror is required to complete it and return it with its offer. The clause at FAR 52-212-4, Contract Terms & Conditions--Commercial Items applies to this acquisition with the following addenda: FAR 52.215-5, Facsimile Proposals, FAR 52.232-19, Availability of Funds for Next Fiscal Year; FAR 52-217.8, Option to Extend Services (with the following blank filled in option by written notice to the Contractor within "60 days"); FAR 52-217.9, Option to Extend the Term of the Contract (with the following blank filled in: (a) "10 days" and "60 days" and (c) "Three years "); and V AAR 852-270-4 Commercial Advertising. The clause at FAR 52-212-5, Contract Terms & Conditions Required To Implement Statues or Executive Orders-Commercial Items applies to the acquisition to include the following clauses: 52.222-21,52.222-26,52.222-35,52.222-36, 52.222-37, 52.232-34, 52.222-41 and 52.222-42. Preventive maintenance of the Circuit Breaker shall be in accordance with industry standards, as well as all applicable Federal, State laws and regulations practices in effect at time of award.
