- This is a combined synopsis/solicitation Request for Quote (RFQ) in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This VA Medical Center is seeking a total rebuild and replacement of the inner mechanics of the Nutrition and Food Kitchen Conveyor System. Contractor shall provide all tools, materials and equipment to replace the inner mechanics and supply and install all moving parts, including Electrical Controls, Electrical Power Wiring, Control Wiring, Rollers, Chains, Sprockets, Shafts, Belts, Bearings, miscellaneous Switches, Solenoids and Relays, excluding housing. The Conveyor System is in two sections and each section will require a wash down drive motor and mounted variable speed, wash protected controller. The sections are approximately 20? and 19? respectively. Work must be completed within 90 days of award. Contractor shall be given 10 consecutive evenings from 6 PM until 5 AM to perform work. A Site visit may be scheduled by calling 202-745-8543. This acquisition is 100% set aside for award to Total Small Business. The NAICS Code is 811219 and the size standard is $6 million. Contractor shall not perform dust producing tasks within occupied areas without the approval of the Contracting Officer. For construction in any areas that will remain jointly occupied by the medical Center and Contractor?s workers, the Contractor shall: Provide dust proof one-hour, fire-rated, temporary drywall construction barriers to completely separate construction from the operational areas of the Dining Room in order to contain dirt debris and dust. Barriers shall be sealed and made presentable on dinging room occupied side. Install a self-closing rated door in a metal frame, commensurate with the partition, to allow worker access. Maintain negative air at all times. A fire retardant polystyrene, 6-mil thick or greater plastic barrier meeting local fire codes may be used where dust control is the only hazard, and an agreement is reached with the Contracting Officer. The Contractor shall adhere to all medical center policies. All contractor employees who require access to perform services at this Medical Center, shall be subject to finger printing by the VA Police Service. This requirement is applicable to all subcontractor personnel requiring the same access. Finger printing will be conducted on the 1st day of the personnel coming on site. Depending on the results of the fingerprinting, a background investigation may be necessary. 1. Contractor Responsibilities (a) The contractor shall prescreen all personnel requiring access to the VA Medical Center. (b) After contract award and prior to contract performance, the contractor shall submit a letter to the Contracting Officer providing the following information: 1. List of names of contractor personnel. 2. Social Security Number of contractor personnel. 3. Home address of contractor personnel. (a) Contractor should provide same list (see b. above) for all subcontractors. (b) Contractor is responsible to update lists as changes in personnel occur. (c) The contractor, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration to work under the contract. (e) Failure to comply with the contractor personnel security requirements may result in termination of the contract for default. 4. Government Responsibilities (a) After receiving the contractor?s list of employees the VA Police Service will fingerprint contractor employees at the Medical Center and issue an ID Badge. Contractor ID Badges must be worn at all times.
