Description
COMBINED SYNOPSIS SOLICITATION (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Submit written quotes (oral quotes will not be accepted) on RFQ reference number F2W3E85234A1. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-03. (iv) This procurement is being issued as a 100% small business set-aside.. The North American Industry Classification System (NAICS) code is 624410 with a small business size standard of (not to exceed) $6,000,000. (v) This combined synopsis and solicitation is for the purchase of Administrative Support Services for Family Member Flight at Malmstrom Air Force Base 341st Services Squadron with the following requirements: CLIN 0001: Statement of Work 1. DESCRIPTION OF AGREEMENT: The contractor shall furnish the following services according to the STATEMENT OF WORK. A. Description of Work. The contractor shall furnish the following services according to the Administrative Support Services for Malmstrom Family Members Program, Family Child Care, statement of work. The length of this contract will be for one year, effective September 2005 through September 2006. B. Service Delivery Summary. (a) Schedules families for the Extended Duty Care and Missile Care Programs. Prepares and submits weekly time cards for providers. (b) Enrolls and registers patrons in need of child care. Assists parents in completing enrollment forms. (c) Maintains waiting list and tracks openings to ensure slots are filled in a timely manner. Contacts families on waiting lists to fill slots. (d) Maintains and updates lending and resource library inventories and catalogs. Assists Family Child Care (FCC) Coordinator with the delivery/pick-up of lending library materials to FCC homes. (e) Assists with other administrative duties such as surveys, mailings, forms, scheduling, etc. (f) Monthly tracks the number of requests for cards, number of placements, number of requests not filled and why, date and hour request was received. (g) Provides assistance five (5) days per week for up to eight (8) hours per day depending on the workload. This includes one evening per week and one Saturday per month for the Lending Library. Should the position require over eight (8) hours on some days, a reduction of hours worked on a subsequent day in that week should be arranged or compensation hours would be accumulated to take off in a week when the work load is not as demanding. (h) The family day care officer will provide at least 24 hours notice to the contractor if services are not required on a particular day. Except in the case of unexpected illness or other emergency, the contractor will provide 24 hours notice of ability to provide service. Such instances are not expected to exceed 3 absences during the course of the contract. Individual will not be paid for referenced absences. (i) Contractor will be provided access to the base and the Government will provide all necessary equipment to conduct the work. Government provided equipment includes, but is not limited to, furnished office space, computer hardware and software to fulfill the duties, office supplies and equipment. (j) Person provided must have current Montana driver?s license, vehicle insurance, and vehicle registration. Must also have proof of current immunizations and physical exam. Must be able to obtain Military Installation Records Check and National Agency Check. (vi) Services requested to start approximately 1 September 2005. FOB destination, Malmstrom AFB, MT 59402. (vii) The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition. (viii) Quotes will be evaluated on price, price related factors and ability to meet specifications. (ix) The offeror must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications ? Commercial Items. (x) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this solicitation. (xi) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders--Commercial Items (June 2004) (DEVIATION), applies to this acquisition which includes: FAR 52.219-8, Utilization of Small Business Concerns (Oct 2000), FAR 52.222-26, Equal Opportunity (Apr 2002)(E.O. 11246), FAR 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans (Dec 2001)(38 U.S.C. 4212), FAR 52.222-36, Affirmative Action for Workers with Disabilities (June 1998)(29 U.S.C. 793), 52.222-41, Service Contract Act of 1965, as Amended (May 1989), FAR 52.247-64, Preference for Privately-Owned U.S. Flag Commercial Vessels (Apr 2003)(46 U.S.C. FAR 52.252-2, Clauses Incorporated by Reference (Feb. 98), FAR 52.233-3 Protest After Award (Apr 1984), FAR 52.233-4 Applicable Law For Breach of Contract Claim (Apr 1984) (xiii) The clause at DFARS 252.232-7003, Wide Area Work Flow applies to this acquisition. The clauses at FAR 52.204-7, Central Contractor Registration (Oct 2003) and DFARS 252.204-7004, Alternate A (Nov 2003) applies to this acquisition. (xiv) All firms or individuals responding must be registered with the Central Contractor Registration (CCR), http://www.ccr.gov/. The Government reserves the right to award on a multiple award or an all or none basis. (xv) Quotes are required to be received no later than 10:00 A.M MST, 1 September 2005.
