Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; propos als are being requested and a written solicitation will not be issued. W912CZ-05-T-0075 is being issued as a Request for Quote, which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-05. This acqu isition is reserved for small business concern. The associated NAICS code for this acquisition is 325188 small business standard size is 1,000 employees. Line Item 0001: Sodium Hypochlorite, Quantity 85 (300 gal tote) STATEMENT OF WORK (SOW) FOR SODIUM HY POCHLORITE (300 gallon tote) Contractor shall provide all transportation and other necessary materials to supply this requirement for Sodium Hypochlorite (12.5% solution strength) for the Water Treatment Plant, Bldg 3565, Fort Wainwright, AK. The contract or shall also pick up empty totes from the same Bldg location when full totes are delivered. A) Approximate consumption will be approximately 13,000 gallons minimum and 15,000 gallons maximum a year. B) The contractor will be notified telephonically when nine (2) empty totes are on hand at the Water Plant, Bldg 3565. Upon notification, the contractor shall within four (4) calendar days deliver three (3) full totes and remove the empty totes. C) All pick up and delivery under this contract will occur bet ween 0800 and 1400 hours Monday thru Friday, and no delivery on Holidays. D) Names of persons authorized to place calls under this contract will be provided in the award. E) Upon initial delivery of the first time 3 (300 gal totes), the Water Plant will p rovide tthree (3) empty sodium hypochlorite totes in exchange for three full totes each delivery thereafter. F) Since empty totes will be exchanged for full totes, it is requested that this contract be offered without a deposit charge for totes. The follo wing provisions in their latest editions apply to this solicitation: FAR 52.212-1, Instructions to Offerors, Commercial. FAR 52.212-2, Evaluation, Commercial Items (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, lowest price technically acceptable and delivery. Offerors shall include a completed copy of FAR 52.212-3, Offerors Representations and Certifications-- Commer cial Items, with their quotes. Offeror shall include a completed copy of DFARS 252.225-7000, Buy American Act Balance of Payments Program Certificate, if applicable. A copy of the Offerors Representations and Certifications and Buy American Act Balance of Payments Program Certificate may be obtained from http://farsite.hill.af.mil.
