- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, 13.5 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitati on; QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number is W81K04-05-T-0019 and is issued as a Request for Quotation (RFQ). This solicitation documents and incorporates provisions and clauses in effect throug h Federal Acquisition Circular 2005-05. This acquisition is Unrestricted Full and Open. The following provisions apply to this acquisition: FAR 52.211-6, Brand Name or Equal; 52.212-1, Instructions to Offerors-Commercial, with addenda: paragraphs (h) and (i) of this provision do not apply. The Center for Health Care Contracting, Ft Sam Houston, TX intends to award a Firm Fixed Price Purchase Order for Brand Name or Equal LiDCO Plus Continuous Hemodynamic Monitors and Stands. Monitors and Stands provided as equals must meet the salient physi cal, functional, or performance characteristics of the LiDCO Plus Continuous Monitor System, to include all hardware and software. All equipment shall be new with manufacturers warranty and shall include shipping costs. Required Delivery Date 30 days after receipt of order. Refurbished units will not be considered. The following Contract Line Items (CLINS) are applicable to this requirement.
