- This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the format in FAR Subpart 12.6 and FAR Subpart 13.5 as supplemented with additional information included in this announcement. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. WRITTEN QUOTATIONS ARE REQUIRED AND A WRITTEN SOLICITATION will not be issued. All prospective offerors are responsible for monitoring this site for the release of amendments (if any) or other information pertaining to this solicitation. Solicitation No. N00604-05-T-0853 is issued as a Request for Quotation (RFQ) incorporating provisions and clauses in effect through Federal Acquisitions Circular 2001-22 and DFARS Change Notice 20040323. This procurement is restricted and is therefore open only to small business concerns. All qualified responsible business sources may submit a quotation, which shall be considered by the agency. Proposals must include a completed Representations and Cer! tifications (FAR 52.212-3 Offeror Representations and Certifications – Commercial Items and DFARS 252.212-7000 Offeror Representations and Certifications – Commercial Items. Please complete your Representations and Certifications on the following website: http://orca.bpn.gov. Contractor must be registered in the Central Contractor Registration Database (CCR) to be eligible for Department of Defense contract awards. Lack of registration in the CCR database will make an offeror/quoter INELIGIBLE FOR AWARD. Please ensure compliance with this regulation when submitting your quote. A single award will be made to the lowest priced, technically acceptable quote. The Regional Contracting Department, Fleet and Industrial Supply Center, Pearl Harbor intends on negotiating a firm fixed price contract for heavy ! equipment repair services, to take place at Santa Rita, GUAM. CLIN 0001 – Stake Truck: replace mirror assemblies; remove, repair, and refit deck; diagnose and repair front end (noise); straighten headache rack; retimber deck; sandblast, prime and paint (green). CLIN 0002 – John Deere Loader: replace radiator grill; replace seat assembly; remove cab and repair rusted areas as required; fabricate panels to repair rusted areas; sandblast, prime and paint CAT yellow. CLIN 0003 – Hyster 3-ton Forklift: replace hood latch; replace rear tires; replace glow plugs, check pre-heat circuit; minor rust repairs; sandblast, prime and paint (color to be specified later). CLIN 0004 – CAT Forklift: sandblast, prime and paint (color to be specified later); replace rusted floor panels. CLIN 0005 – Backhoe: replace seat assembly; replace gauge cover; replace rusted and damaged hoses; replace rusted tool box hinges; prep, sandblast, prime and paint CAT yellow. All CLINs are separately pric! ed; please provide individual pricing for each, FOB Santa Rita, GUAM. Performance date: no later than 28 February 2006, to be coordinated between Contractor and Activity.
