Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Requisition number F2P3E15216A100 is issued as a Request for Quotation (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-04. This acquisition is unrestricted: STATEMENT OF WORK 1. DESCRIPTION OF SERVICES: In support of the Furnishings Management Office and Unaccompanied Housing (FMO UH), the Contractor shall provide ?First class preventative maintenance service of 97 US washing machines and 119 US Tumble dryers located in 13 US Government dormitory laundry rooms?. In the performance of this service the contractor shall expect to encounter frequent interfaces with a variety of authorized Government personnel and/or their representative(s). The service shall include a maintenance check on all washers and dryers once every 90 days. The contractor shall as a minimum be expected to perform all testing, stripping down of machines for access to vital areas, lubricating of parts, and cleaning to meet commercial standards. All work carried out under the SOW shall have a minimum guarantee of two months. 2. MANAGEMENT: The Contractor has overall responsibility to ensure all personnel, protective gear, tools, supplies, supervision, equipment, automated systems and other items and services necessary to successfully accomplish contract requirements. It is the Contractor?s responsibility to ensure all employees are fully trained, professional and customer service orientated. The service provider shall ensure high-quality work performance in accordance with applicable standards and guidelines. 2.1. The Contractor shall provide a point of contact (POC) that shall have full authority to act for the Contractor on all matters relating to the operation of this contract. The name of this person and alternate(s), who shall act for the contractor when the primary POC is absent, shall be designated in writing to the contracting officer prior to the contract start date. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This shall be accomplished through the use of a standardized and distinctive article of clothing or accessory, such as a shirt, badge, or hat with company name on it. The Contractor shall ensure employees have valid identification, passes and certifications, as applicable, to accomplish the service of the SOW. 3. DRYER SERVICE: The Contractor shall establish and maintain a proactive preventative maintenance program. Disconnection of appliances must meet or exceed National Inspection Council for electrical Installation Contractors. The minimum service shall consist of inspections for any running faults, removal of lint from exhaust system and internal electrics, oiling of all parts as required, machine inspection, final full functional test and replace vent hoses when required and reinstalled, according to the manufacturers manual and the Institute of Electrical Engineers (IEE) regulations. Appliances may need to be stripped down in order to gain access to vital areas. Appliances found with faults are to be reported to the FMO within 2 hours. The contractor shall also provide all necessary protection to keep the appliances and laundry rooms free from damage. After completion of service, contractors shall leave the laundry room service area and appliances clean and free from any debris. The contractor shall also be responsible for any faulty installations. 4. WASHER SERVICE: The Contractor shall establish and maintain a proactive preventative maintenance program. The minimum service shall consist of a full cycle check, inspection, clean including lime scale removal and full functional test upon completion. Appliances may require stripping down in order to gain access to vital areas such as motor, agitator, filters etc. Appliances found with faults are to be reported to the FMO within 2 hours. The contractor shall also provide all necessary protection to keep the appliances and laundry rooms free from damage. After completion of service, contractor shall leave the laundry room service area and appliances clean and free from any debris. The contractor shall also be responsible for any faulty installations. 5. GENERAL INFORMATION: Schedule Upon award the FMO will provide a detailed timetable showing all necessary dates, times and locations which should be adhered to during this contract period. 5.1. BUSINESS HOURS: FMO maintains the following customer service hours: 0800- 1600, Monday through Thursday, and 0800-1500 on Friday. GOVERNMENT QUALITY ASSURANCE: A Government representative will assess the Contractor?s performance through intermittent on-site inspections. The representative will require the contract manager or the Contractor?s representative to acknowledge any defective service identified and initiate remedial action as required. 5.3. GOVERNMENT REMEDIES: In the absence of negotiation remedies, the Contracting Officer shall follow FAR 52.212-4, Contract Terms and Conditions for Contractor?s failure to perform satisfactory services or failure to correct non-conforming services. 6. GOVERNMENT REQUIRED FORMS FOR DOCUMENTATION OF PROCESSES: Service Call Sheets - To be used when inspections and maintenance has been carried out on washers and dryers. Service Sheets - Details all aspects of work carried out on major system service. All documents will be provided by the contractor to the FMO upon completion of work within 5 working days. 8. GOVERNMENT PROPERTY ITEM DESCRIPTION: The type, size and weight of items may differ according to FMO supplies during the course of the contract. What follows is a general description of the typical items of appliances. US Washer (Whirlpool) US Washer (Speedqueen) US Dryer (Whirlpool) US Dryer (Westinghouse) US Dryer (Speedqueen) This will be a one year contract with two option years. FAR provision at FAR 52.211-6 Brand Name or Equal, applies to this requirement. If quote is for an ?equal? product, specifications of the item must be included in quotation. FAR provision at FAR 52.212-1, Instructions to Offerors- Commercial, applies to this RFQ with the following addenda: paragraph (i) of this provision does not apply and change paragraph (c) from 30 calendar days to 60 calendar days. Each offer submitted will be evaluated separately. Far provision 52.212-2, Evaluation ? Commercial Items, applies to this RFQ with the following addenda: Paragraph (a) is filled in with (1) Technical Specifications (2) Past performance (3) price, (Technical specifications and past performance, when combined, are approximately equal to price). Offerors shall include with the submission of their offer a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications?Commercial Items. The clause at FAR 52.212-4, Contract Terms and Conditions- Commercial Items, without addenda and 52.212-5 Contract Terms and Conditions Required to Implement Statues of Executive Orders ?Commercial Items, without addenda, applies to this acquisition (including: 52.222-21, 52.222-26,52.222-35, 52.222-36, 52.222-37 and 52.232-33).
