- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-05-T-0280. The acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effec t through Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is: 621999. The requirement is unrestricted. Lyster Army Health Clinic, Fort Rucker, Alabama 36362 has a requirement for Nu rse Triage Call Service. The requirement is required to meet Health Affairs requirements for primary care manager (PCM) access by telephone or appointment 24-hours per day; seven days a week to address acute care that is non-emergency needs which often. T he service shall support approximately 3000 families equal to 12,000 individuals enrolled to a PCM at the Lyster Army Health Clinic as well as 5800 active duty members. This service will be provided 24 hours a day on weekends and holidays and from 4:00 PM to 7:30 AM weekdays. This service will be provided by experience registered nurses using state-of-the-art computer based triage guidelines that are base upon algorithms developed by recognized consultants such as Barton Schmitt, MD for pediatrics and Micro medex for adults. Whatever consultant algorithm proposed must be approved by the Lyster Army Health Clinic commander prior to acceptance. The guidelines must lead the nurse to identify the severity of the symptoms and advise the most appropriate level of c are for the caller. The levels of care include home treatment and education, appointments at Lyster Army Health Clinic for routine or same day care, appointments at Network facilities for urgent acute care, and Emergency Room referrals when emergency trea tment is needed. At a minimum there will be guidelines for adults, pediatrics, over the counter medications, and dosages. Lyster Army Health Clinic enrolled beneficiaries and active duty military member calls seeking this service will be answered by a liv e voice within 20 seconds by a service representative. Calls are triaged as follows: 911 criteria; RN immediately; Routine; Health or clinic information. Patients will not be put on hold, all calls will be stored for future use, and documentation managemen t will be HIPAA compliant. Bidders may propose affixed price for service or a fee for service. If fee for service is proposed then the resultant contract will be for PRIME (enrolled to facility) patients only plus our active duty military. The government may provide a computer access to identify enrolled beneficiaries. The providers of this service will be expected to develop a quality assurance plan (QAP) to self monitor services. This plan shall include, but not limited to, time caller waits for be answe red, total time of service, patient outcome, and appropriateness of call-backs to patients, HIPAA compliance, and review of documentation for appropriateness. The period of performance for this service is for 01 October 2005 through 30 September 2006 plus 4 one-year option periods. Proposed rates shall be in accordance with the Service Contract Act, Wage Determination No: 1994-2005, Rev. 23 Area: Augusta, GA. FAR 52.212-2-Evaluation-The Government will award a contract resulting from this solicita tion to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Evaluation Factors: (Technical Capability and Qualifications- Offers will be considered only from co ntractors who are licensed to perform services, documentation is required. (2) Past Performance The contractor should describe the past experience in providing the services specified in this solicitation within the past three (3) years. (4) Price (Total price for Base Period including Option Year. Past Performance, Technical Capability, Qualifications, and Availability when combined are of equal importance when compared to price. The Government reserves the right to determine what constitutes approximately equal in technical capability and past performance. The Government reserves the right to evaluate offerors prices on the basis of price realism and will consider the offerors price as an indication that they understand the scope of the Gove rnments requirement.
