- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The solicitation number is RFQ 246-05-02266. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-04. The NAICS is 423450 and the small business size is 100 employees. THIS IS A TOTAL SMALL BUSINESS SET-ASIDE. Items will be purchased and delivery will be made to the Department of Veterans Affairs Medical Center, Supply Building 27, Averill Drive, Hampton, VA 23667. Offeror’s proposals are hereby being requested in response to the requirements under firm-fixed price contract and shall be submitted on an offeror’s letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. Quotes are to be provided for eight (8) SHOWER/COMMODE WHEELCHAIRS in strict accordance with the following specifications: (1) Metal rigid frame, with open-style side frame for accessibility and safety, (2) 5” casters, (3) Wheels lockable, all four, foot activated brake system to ensure chair does not move when engaged, (4) Multi-positional, reclining, with positive locking mechanism, (5) Under seat clearance, at least 23-1/2”, to fit over 21”ht X 22” wide commode, (6) Padded arm troughs, (7) Arm Rests on outside of frame, (8) Leg Rest: Extra long, adjustable length (without tools or wing nuts), elevating, multi-positional, swing away and removable with single easy motion, with swing-away, padded calf support, (9) Padded sculpted foot pedals, (10) Foot rest matches seat width, rather than frame width, (11) Chest and leg straps, adjustable 2” velcro, 72 inches (36” each side), attaches by wrapping around frame and going through clip (removable), (12) No more than 18” distance between inferior edge of “hangers”, (13) 18” square thick cushion, set on inner mount, positioned so back of patient legs DO NOT contact metal framework. Cushion 2-3/4” thick to 4”, (14) Padded backrest and padded adjustable headrest, (15) Non-white upholstery/foam, (16) Water resistant frame, padding, foam, (17) Support a person weighing 350 pounds. Quotes shall be valid for (30) days from the offeror’s due date. All responses must include the following: (1) descriptive literature and product number for the item offered providing the product offered meets the above requirements (2) warranty information (3) cost of product (4) shipping charges (5) delivery time after receipt of order. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2, Service of Protest, 852.233-70, Protest Content, 852.233-71, Alternative Protest Procedure. Award will be made to the offeror whose product meets the required specification as listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. The following factors shall be used to evaluate offers: (1) technical capability, (2) pass performance, (3) delivery, and (4) price with technical capability being superior to price. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-3, Offeror Representations and Certifications-Commercial Items. In accordance with FAR 52.204-7 award will be made only to a contractor/company that is registered in the Central Contractor Registration (CCR). Electronic copies of the referenced provisions and clauses are available on the internet at www.fedbizopps.gov. Quotations and all required information must be received by 12:00pm local time September 9, 2005. Cut off date for all questions will be September 6, 2005 and must be submitted in writing. Quotations shall be submitted in writing to Ruth B. Harris, Contracting Office (90C), VA Medical Center, 27 Averill Drive, Hampton, VA 23667. No fax or electronic offers will be accepted. Solicitation number RFQ 246-05-02266 shall be listed on the documents submitted for identification purposes. Only offers prepared in accordance with this solicitation shall be considered.
