Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W81K00-05-T-0897 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisiti on Circular 2005-05. This acquisition is solicited on an unrestricted (full and open) basis under NAICS code 339113 and the size standard is 500 employees. The requirements of this solicitation are: CLIN 0001, 1 each ETHICON (JOHNSON AND JOHNSON) HARMON IC SCALPEL GENERATOR PART NO. GEN04, and CLIN 0002, 1 each ETHICON GENERATOR CART PART NO. CRT01, and CLIN 0003, 1 each ETHICON FOOT SWITCH AND CABLE PART NO. FSW01, and CLIN 0004, 2 BOX (1 EA/BOX) ETHICON HAND PIECE PART NO. HP054. Federal Acquisition Re gulation (FAR) Clause 52.211-6 Brand Name or Equal applies to this acquisition. Award will be on an all or none basis. CLIN 0001 salient characteristics are: Hand/foot activated, built-in system diagnostics, easy to read control panel, dual foot switch r eceptacles, 110V, CLIN 0002 salient characteristics are: Shall be compatible with ETHICON CRT01 GENERATOR CART, CLIN 0003 salient characteristic are: Shall be compatible with ETHICON FSW01 FOOTSWITCH AND CABLE, CLIN 0004 salient characteristics are: Dig ital core smart chip defines output power curve, transducer converts electrical energy from the Generator into mechanical energy (ultrasonic vibration). Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.2 11-6 (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB Destination to U.S. Army Medical Activity, Centralized Instl and Org PBO, Attn: Karan Bilyeu, 7500 Cochrane Circle, Fort Carson, CO 80913-4604. FAR provision 52.212-1 [Instructions to Offerors  Commercial] applies to this acquisition to include the following addenda: All Offerors are cautioned that if selected for award, the y must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the Internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals wi ll be evaluated in accordance with the following criteria: Technical Capability (compliance with the requirements of FAR Clause 52.211-6 if vendor offers an Equal Product) and Price, with Technical Capability being significantly more important than Price. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation.
