Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this requirement is RMK0E050054 and is issued as a Request for Proposal (RFP). The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-18. This requirement is set aside for small business with a NAICS Code of 611710 and the business size maximum is $12.0 million. The performance period for this requirement is from September 19, 2005 through May 26, 2006. This office plans to award a contract to a contractor that can provide the following educational support services in a school setting: 1) Physical Therapy, 2) Behavior Therapy/Counseling Services, 3) Speech/Language Therapy, 4) Occupational Therapy, 5) Hearing Impaired Therapy, 6) Vision Impaired Therapy, 7) Transition Specialist, 8) School Psychologist/Evaluator. All therapists must be certified and must provide the certification in the proposal. These services are to be performed at the following schools within Chinle Agency: Chinle Boarding School, Jeehdeez?a Academy, Pinon Community School, Rock Point Community School. All of these schools are located in Arizona. Contractor shall provide these services for students who have been referred for such services by their respective schools in the Chinle Agency in accordance with P.L. 105-17. The number of days per week could change (increase or decrease) during the course of the school year, depending upon number of students placed for these services. These services will be provided to the students requiring services enrolled in special education. The contractor will complete a comprehensive assessment of initial referrals and the three year reevaluation of students with needs, write the assessment report using the Chinle Agency Evaluation report format, chair the initial staffing, chair the MDT/IEP Team to develop individualized education program for the initial speech identified students and for the students who are already in the program. Work Day: The work day will consist of an eight (8) hour work day and will arrive to the respective school at 8:00am and leave the school building at 4:30pm with a 30 minute lunch break or at 5:00pm with an hour lunch break. The Therapist must sign in at the main front desk, at the Special Education Department Visitors Log in Sheet and also sign the Related Service Documentation binder. The same procedures/steps must be taken when completing the work day. After 15 minutes of missed work time, the daily rate will be divided up by eight hours and deducted from the daily rate of pay. 40.5 cents per mile will be calculated on a per/day round trip basis from the related service provider?s home to the scheduled school-site. Contractor will use POV and be reimbursed for all travel from home to worksites at the rate of .405 cents/mile. Therapy services will not exceed May 26, 2005. An interpreter will be used when testing students whose primary or dominant language is the Navajo language. Assessment reports will be comprehensive, individualized and meaningful for parents and educators. The report will include, but not be limited to reasons for referrals, in-depth background/development/social/academic and medical histories, observation, parent and teacher interviews, teaching and intervention strategies, placement recommendations, and summary of scores. Conduct staffing to interpret the assessment findings to the parent and the MDT regarding the most appropriate instructional program and placement for the student, provide training sessions for the staff and parents, provide inclusion services for the students enrolled in the special education program, attend and chair meetings pertaining to students requiring these services. The evaluator and the special education teacher will develop a schedule to provide services and inclusion services to students. Progress Reports Progress Reports for each student receiving services are to be completed utilizing the electronic IEP forms by Standard Design. Progress reports are to be completed five (5) days prior to the end of each quarter of the respective schools. It is the Therapist?s responsibility to make adequate copies for the file, parent, pertinent general education staff, and for other required purposes. The Therapist will prepare for mailing the progress reports to each parent, for each student. IEP Forms All IEP information will be completed on electronic Standard Design 2005 forms. Hand written information will not be acceptable. Chinle Agency Trainings The Therapist will attend all trainings relative and pertinent to providing direct services with children requiring services. Written Documentation It is the responsibility of the Therapist to provide concerns or complaints in written form to the school?s respective Principal/Executive director and the Chinle Agency special Education Coordinator. It is the responsibility of the Therapist to document each child?s progress on the documentation sheet provided by the individual school. This contract will use Federal Acquisition Regulation (FAR) clauses. The following clause 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Offerors are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their proposals along with a DUNS Number. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statures or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33. Required Central Contractor Registration and Department of the Interior Acquisition Regulation (DIAR) 1452.226-70 Indian Preference, DIAR 1452.226-71 Indian Preference Program, DIAR 1452.204-70 Release of Claims are applicable.
