Description
The Uniformed Services University of the Health Sciences (USUHS), Bethesda, MD requires two (2) sterilizers (single door) to include installation and the removal/disposal of one existing unit. The contractor shall provide all personnel, equipment, tools, materials, and transportation to install and setup the two (2) new sterilizers. SPECIFICATION: Sterilizer, 24?x 36?x 48? (internal chamber dimensions), single door; 316L stainless steel chamber, jacket, and door construction; manual hinged radial arm door with safety interlocks; non-proprietary components for all plumbing, electrical and control systems; vacuum and gravity cycles for the sterilization of liquids, porous loads, and hard goods; and steam operated; one unit requires a recessed installation with vermin seal; and the other unit requires a free-standing unit. INSTALLATION: The contractor shall be responsible for the movement of all equipment to the installation site. The contractor shall install the units complete with the components. The contractor shall also perform verification testing and provide technical support. Installation shall be completed within 30 days after delivery to USUHS and is to be coordinated with the COTR (to be named at time of award). SET-UP and TRAINING: The contractor shall be responsible for performing all start-up operations to ensure the units are fully functional for its intended purpose. The contractor will provide at least three full days of operator training at USUHS within 15 days after completion of installation. DOCUMENTATION: Upon completion the contractor shall provide two (2) sets each operation and service literature. Service literature will illustrate all wiring diagrams, schematics, and parts lists with all part numbers and assembly breakdowns. Literature is needed for future in-house service and repair only outside of the warranty period. WARRANTY: Warranty will be for one year from date of installation to include 100% parts and labor. Warranty response will be on-site. Warranty repair will be performed by a locally based, factory trained, qualified technician and will respond within 48 hours of a service call. Warranty is available at no additional cost to the Government. DISPOSAL: Contractor will remove and dispose of one (1) unit. The sterilizers must fit through a standard 36? door opening (34 ?? jamb spacing). This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Award is based on FAR 13, Simplified Acquisition Procedures utilizing best value analysis. This announcement constitutes the solicitation; quotes are being requested and a written solicitation will not be issued. Amendments and/or additional information will be posted on the FedBizOpps, www.eps.gov. As a reminder, offerors are responsible for monitoring the website for possible amendments and/or additional information. This combined synopsis/solicitation is a request for quotes (RFQ). The incorporated provisions and clauses are those in effect through FAC 2005-05. The NAICS code is 339111. This notice is being used as request for quote for: (0001) SINGLE DOOR STERILIZER: 20? x 20? x 38? INTERNAL DIMENSIONS, 316L STAINLESS STEEL CHAMBER, u/i: 2 EACH. The Government reserves the option to purchase additional quantities at the same price as the like items called for in this solicitation. The following provisions apply to this acquisition and are required with your proposal: FAR Clause, 52-212-1, Instruction to Offerors - Commercial Items (JAN 2005), FAR Clause, 52.212-3 ? Offeror Representations and Certifications ?Commercial Items (MAR 2005), and the evaluation factors outlined in FAR Clause, 52.212-2, Evaluation ? Commercial Items (JAN 1999), and the acknowledgment of amendments (if any), 52,217-6, Option for Increased Quantity (MAR 1989). Telegraphic or facsimile offers are acceptable. Period for acceptance of offers is amended to include the following: The offeror agrees to hold the prices with its offer firm for 30 calendar days from the date specified for receipt of offers. The provision of FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The following evaluation criteria are included in paragraph (a) of the provision: Any resulting order will be issued to the company offering the best value to the Government, price and other factors considered. Factors that will be considered are Technical capability and price. Failure to provide the information relative to each evaluation may render offer non-responsive. 1) Technical Capability: Technical capability will be based on product literature, technical features and/or warranty provisions. Each offeror is required to submit descriptive literature, or other documentation, manufacturer or brand name and model of the item, and show how the offered product meets or exceed the requirements as specified in the Specifications; 2) Price: Price includes all equipment, software, training, warranty costs, discount terms, and transportation. Evaluation is based on best value including cost and ability to meet the stated requirements above. Offerors shall submit descriptive literature and drawings detailing features, technical capabilities and warranty data. Evaluation will be determined solely on the content and merit of the information submitted in response to this requirement. Therefore it is essential that offerors provide sufficient technical literature, documentation, etc. in order for the Government evaluation team to make an adequate assessment of the quote as meeting technical acceptable. The clauses listed are not all inclusive for the firm-fixed price contract and additional clauses applicable to the fixed price contract may be added at time of contract award or any other clauses the Contracting office deems applicable to the effort.
