Description
This is a combined synopsis/solicitation. Request for Quotation #RFQ 69D-085-06 entitled ?EKG Technician Services? is hereby set aside for service disabled veteran owned small businesses and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2005-05 and VAAR 97-10. NAICS Code 621999 applies for this procurement and the small business size standard is $8.5 million. This announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor?s proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor?s letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Contractor?s price and technical proposal must contain the name; title and signature of person authorized to submit proposals/bids on behalf of the contractor. Jesse Brown VA Medical Center, 820 S. Damen Avenue, Chicago, IL has determined a requirement for one (1) (FTE) EKG Technician to work Monday through Friday 8:00 am to 4:30 pm for the period from September 12, 2005 through November 15, 2005, with one (1) 45-day option to extend. Contractor to provide services of qualified and experienced EKG Technician who will be required to perform Electrocardiographs and Holter monitoring on outpatient and inpatient veterans. This will consist of collection of patient data, teaching, and application of electrodes, scanning results, and editing data from the reports during Holter monitoring. The Tech will independently scan Holter monitor tapes using the computerized scanner system. They must have a good technical background in arrhythmia recognition and be able to differentiate between premature ventricular contractions, supraventricular premature contractions and artifacts. This knowledge becomes critical on those occasions when a patient exhibits certain abnormalities and then needs to be referred to a cardiologist immediately. The Tech must also have various bedside techniques when testing patients on orthopedic, intensive care, psychiatry wards, and the ER. The Tech must be able to operate a single channel and three channel EKG machine. All work is to be performed in accordance with all requirements contained in VA Handbook 7176 and in accordance with JCAHO standards. Must be able to work independently with minimal supervision. The contractor must provide the following documents for each contractor employee working under this contract: credentials (certificate) and qualifications for the job; a current competence assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform this job correctly and skillfully; a current performance evaluation supporting ability of the contractor employee to successfully perform the work required and a listing of relevant education.) A full-time equivalent (FTE) is defined as a contractor's employee working eight (8) hours per day, five (5) days per week (40 hours per week). Contractor's employees must be present at VA and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A VA record-keeping system of contractor hours worked is established through time and attendance logs in the departments being served. Payments to contract personnel for any leave, including sick leave, holiday, or vacation time, are the responsibility of the contractor. Leave granted to contract employees would be in accordance with contractor policies. VA is not obligated to the Contractor during technician?s absence. Evaluation of proposals offered will include the following factors: Professional qualifications, immediate availability, past performance and price. Award will be made to the acceptable candidate determined to be the ?best value? to the Government. Evaluation may be made with or without discussions with the offeror(s). All responsible sources that can meet the requirements and provide services as listed above may respond to this solicitation by submitting cost and technical proposals (curriculum vitae) on their company letterhead or bid form.
