- AMENDMENT 001 ISSUED TO ADD AND DEFINE QUARTERLY SALES REPORT DUE TO THE VA AS PART OF THIS POTENTIALLY AWARDED CONTRACT. The Contractor shall provide a quarterly report of sales for each medical center by line item, no later than 60 calendar days after the end of each calendar quarter ending March, June, September and December. Reports shall be sent electronically to the contracting officer at Timothy.Scanlan@med.va.gov . In order to track compliance, VA requires the contractor to submit the quarterly report of sales in the attached prescribed format. At time of award, VA will provide the contractor with an electronic copy of the template that shall be used for all quarterly report of sales. The contractor agrees by submitting a quotation that he shall adhere to this requirement. In addition to direct sales with VA facilities, the Contractor shall also include in the quarterly sales report any sales made under VA?s Medical/Surgical Prime Vendor Program. Prime Vendors may charge you a fee for keeping track of the sales on your contracted products. ALL OTHER TERMS AND CONDITIONS OF THIS SOLICITATION REMAIN THE SAME
