- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Womack Army Medical Center Contracting Office intends to award a contract to Duke Medical Center for Pediatric Cardiologist. Contractor shall provide to the Chief, Department of Pediatrics, Womack Army Medical (WAMC) non-personal services as a qualified m edical or physician to provide pediatric cardiology consultation services in accordance with the terms and conditions of this contract. The contract physician shall provide these services to all eligible beneficiaries of the military health care system wh o present themselves to WAMC for treatment. The Contract physician shall be of a quality or exceeding current recognized national standards as established by the Joint Commission on Accreditation of Healthcare Organizations (JAHCO). Contractor shall comp ly with medical staff rules, regulations, and bylaws related to practice as promulgated by the Army and Commander, WAMC. This solicitation is unrestricted. NAICs code is 622111. Contract shall be issued for period of performance 1 October 2005 30 Sept ember 2006. Qualifications: Allopathic or osteopathic physicians utilized by the contractor to fulfill the terms of this contract must be licensed to practice medicine in the State of North Carolina. Only residency trained or board certified pediatric ian or cardiologist, currently licensed to practice medicine in the State of North Carolina shall render the final interpretation of an electric cardiogram. The contract physician providing services shall be able to read, write, and speak English in order to effectively communicate effectively with patients and WAMC staff. The Contractor shall ensure contract HCPs have received Occupational Safety and Health Administration (OSHA) required training for the position in which they will perform, and that tra ining is maintained current. Documentation of such training shall be provided to the Contracting Officers Representative (COR) upon contract Award. Current certification in Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) is re quired. The contract surgeon shall be responsible for obtaining and maintaining the necessary recertification. While providing services under this contract the contract cardiologist shall maintain a current state medical license in at least one state in t he United States. The contractor surgeon is required to comply with any applicable state laws concerning their profession. Certification/Recertification: It is a condition of this contract that all certification required elsewhere in this statement of wo rk remains current during the life of the contract. No alien shall be employed who does not have a valid U.S. Immigration Form 1-151, Alien Registration Card. The Contractor shall not employ any alien in the performance of work under this contract in vi olation of the Immigration Laws of the United States. The contractor shall require such documented evidence of his/her/ employees and take such other action as is reasonably necessary to insure compliance with this provision. HOURS OF PERFORMANCE: Hours of service shall be between the hours of 0900 1500, one or two days per month. The days of service shall be coordinated with the contractor 30 days in advance. Contract pediatric cardiologist shall perform the following tasks: Provide basic physical exams and histories of both inpatient and outpatient referred to the WAMC Pediatric Service. Provide expert advisory services in the specialty of pediatric cardiology to staff providers at Womack Army Medical Center to include personal views on problem ca ses within the contract providers scope of specialized training and experience. Conduct diagnostic studies, such as echocardiograms, on patients referred to pediatric cardiology. Examine all pediatric patients, newborn through adolescent referred for cardiology medical care to render a medial assessment of their condition and to take the ap propriate action. Consult with other WAMC staff specialties as appropriate to determine referral priorities of patients whose conditions are diagnosed as beyond the capabilities of the healthcare provider or clinic. Prepare referrals thoroughly documenti ng the patients condition, history and reason for referral. The contract physician shall become familiar with the WAMC formulary. The contract physician authorized to prescribe pharmaceuticals shall do so according to the availability of drugs listed t herein. The pharmacy service will provide instructions to all prescribing practitioners on substitutability of generic drugs for prescribed drugs. The contract physician shall only use prescription forms DA1289, which have been stamped To be filled only at WAMC. In addition to price, offeror are to provide information in a separate document titled Technical Qualifications to establish offerors professional qualifications and capability to provide and retain the required services. Closing date is 15 September 2005, 1700 PM EST. Addendum to FAR 52.212-1, Instructions for Offerors will provide price, travel costs, if required, past performance and Technical Qualifications. Technical evaluation must provide how the pediatric cardiologist will provide c ontinuity of services with patients. In accordance with 52.212.2, proposals will be evaluated on price, past performance, and technical qualifications.
