Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation FA4877-05-Q-0492 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2004-24. (iv) This procurement is being issued as a 100% SMALL BUSINESS set-aside under NAICS code 562910 and small business size standard $12M. (v) The contractor shall provide all tools, equipment, personnel necessary to remove and dispose of flooring associated adhesive and carpeting from rooms #101 thru #140 in building #140 at Davis-Monthan AFB, AZ. Work hours 0700 to 1600 hours (weekend work acceptable if required.) Set up (HEPA) filtration equipment and associated containments. Remove by hand (non mechanical methods) complying with all Federal, State and local requirements. Interior items to be removed and replaced by others. This not included in the contract. Adhesives to be removed with biodegradable remover and furnish an MSDS for DMAFB contracting and Civil Engineer Squadron. Provide final air sampling with third part analysis. Allow 5 crew days for mobilization, removal and sampling. All work to be done in accordance with the Statement of Work. All interested parties must contact the contract administator listed below by no later than 9 Sep 05, 1600 PST to receive a copy of the Statement of Work and to be scheduled for a one time site visit to be determined at a later date. (vi) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (vii) 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on price. (viii) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, or complete on ORCA website: http://orca.bpu.gov/ with its offer. (x) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
