Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-05. All applicable provision and clauses are included in the actual solicitation. This procurement is set for full and open competition. The North American Industry Classification System (NAICS) code is 813110, and the small business size standard is $6,000,000. The Government intends to issue a firm fixed price purchase contract to cover a period of 1 Oct 05 - 30 Sep 06, to include two (2) option years, for the following: CLIN 0001: Catholic Mass Music Director STATEMENT OF WORK TITLE: Roman Catholic Choir Director/Guitarist LOCATION: 1100 Sunday Mass Capehart Chapel Offutt AFB, NE 68113 I. QUALIFICATIONS: 1. Must be a proficient instrumental musician on acoustic guitar. 2. Must be able to read music, be familiar with musical nomenclature, and have knowledge of music theory. 3. A Bachelor?s Degree in music or a related field is highly desired. As documented by a copy of the degree. 4. Successful experience as a Music Coordinator or Choir Director in church or were sacred music is highly desired. As documented by work references 5. Must be thoroughly familiar with Roman Catholic Liturgy and be competent with the various liturgical ministries as well as other rites and sacramental celebrations. 6. Must be capable of selecting, recommending, and presenting instrumental and/or choral music relating to the liturgical season, rite, or sacrament being celebrated. 7. Must possess sufficient interpersonal skills to facilitate a harmonious and successful Working relationship with a Catholic Liturgical Committee. As documented by work references II. DUTIES 1. Will provide own transportation to and from work. 2. Will remain current with respect to Roman Catholic Liturgical and musical requirements and implement changes as required. 3. Will arrive no later than 30 minutes prior to each service he/she is scheduled to attend and 15 minutes prior to rehearsals to allow for preparation of cantor, musicians, choir, and any other details which may arise. 4. Will be appropriately attired for all Catholic worship services. 5. Will be responsible for selecting, filing, and submitting music Fund Request as needed. 6. Will attend and participate in Catholic Liturgy Committee meeting. 7. Will, as determined by the Director, either lead the choir and congregation personally, or will ensure a qualified substitute is available for Catholic Cantor and Choir rehearsals at the Chapel and for Catholic Masses on Sunday, 1100 (Capehart Chapel) and for necessary rehearsal. Each rehearsal will last one hour. 8. Will, as determined by the Director, either lead the choir and congregation personally, or will ensure a qualified leadership replacement is available for all Catholic Holy Days, Holy Week, and special services to include, but not limited to: Advent, Christmas, First Communion, and Confirmation, plus necessary rehearsals for these services. 9. Will provide music for all Catholic Masses on Sunday Obligation, Holy Days, Holy Week and special services to include, but not limited to: Advent, Christmas, First Communion, and Confirmation, plus necessary rehearsals for these services. 10. Will select, recommend for the approval for the Senior Catholic Chaplain, and prepare music selections for all applicable Catholic Masses and services. 11. If unable to supply for a designated service and/or rehearsal, Director will provide 24 hours notification of the Senior Catholic Chaplain. The Director will provide a qualified substitute for the service/rehearsal and will be responsible for payment of the substitute. (It is assumed the Director will include the substitute?s Units on the end-of-month payment invoice in order to render payment to the substitute.) 12. Will be given 24 hours notification of cancellation of worship services. If 24 hours notification is not made, Director will receive payment for said services. 13. Will not receive payment for any supply of services not specified in this statement of work. 14. Will submit invoice of services and rehearsals provided during the month, no later than the 25th day of the month. 15. Will be directly responsible for securing chapel facilities when used after normal duty hours. 16. The chapel reserves the right to change the work schedule provided the Director is given notice one week prior to the change. III. CRIMINAL BACKGROUND CHECK REQUIREMENT. 1. The Contractor will conduct National Agency Check criminal background checks on all personnel providing services under this contract using SF85P, written and electronic, meeting the formal requirements of the investigation for criminal background checks. Background checks must be completed prior to personnel starting work in the Chapel . Evidence of such submission must be included in the initial credentialing packets. State criminal history background checks will also be conducted, the results of which will also be included in the initial credentialing packet. 2. With the written approval of the 55 WG/HC Security Manager, contract personnel may provide contract services prior to completion of background checks. However, at all times while children are in the care of that individual, the contract personnel shall be within sight and continuous supervision of a staff person, whose background check has been completed, a chaperone, or parent/guardian. 3. Contract employees shall have the right to obtain a copy of any background check pertaining to them and to challenge the accuracy and completeness of the information contained in the report. 4. Contract employees who have previously received a background check shall provide to the QAP proof of the check or obtain a new one.
