Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-05. All applicable provision and clauses are included in the actual solicitation. This procurement is set for full and open competition. The North American Industry Classification System (NAICS) code is 813110, and the small business size standard is $6,000,000. The Government intends to issue a firm fixed price purchase contract to cover a period of 1 Oct 05 - 30 Sep 06, to include two (2) option years, for the following: CLIN 0001: Protestant Praise Team Leader STATEMENT OF WORK TITLE: Praise Team Leader LOCATION: 1900 Saturday Contemporary Worship Service Capehart Chapel Offutt AFB, NE 68113 I. QUALIFICATIONS A. Have five years experience using traditional and contemporary Christian music in a worship environment. Must be documented by work references B. Have working knowledge to set up and work with the MAKIE TT 24 Digital console or similar digital technologies and associated sound equipment used for the 1900 Saturday Contemporary Worship Service. C. Have ability to conduct rehearsals with instrumentalists and vocalists from the worship team, as well as other participants in rehearsals and the worship services. D. Must work in and be fully conversant with the pluralistic environment of the USAF Chaplain Service. II. DUTIES A. Select music from traditional and contemporary Christian music sources that will enhance the worship experience. Final approval on all selections rests with the chaplain in charge of the service. B. Prepare and coordinate Power Point slides of all selected songs for each service. C. Lead congregational singing during each worship service, assisted by the worship team. D. Recruit and schedule participation by other soloists, ensemble groups, or instrumentalists as necessary and appropriate. The chaplain in charge of the service must approve the participants. E. Schedule one rehearsal each week with the members of the worship praise team. Rehearsals with other participants will coincide with the regular rehearsal and will not be considered an extra service. F. Perform all listed services agreed to herein or provide and reimburse a suitable substitute. The chaplain in charge of the service must approve all notices of substitution. G. Provide, in writing, to the chaplain in charge of the 1900 Saturday Contemporary Worship Service, the total number of services and rehearsals performed each month no later than the 25th of that month. Payment of services will be made within 30 days of receipt of invoice. H. The period of performance is 1 October 2005 through 30 September 2006 and covers a total of 52 units for the regular service requirements, 52 units for rehearsals and 6 additional units for miscellaneous services agreed upon by the Leader and chaplain. III. CRIMINAL BACKGROUND CHECK REQUIREMENT. 1. The Contractor will conduct National Agency Check criminal background checks on all personnel providing services under this contract using SF85P, written and electronic, meeting the formal requirements of the investigation for criminal background checks. Background checks must be completed prior to personnel starting work in the Chapel. Evidence of such submission must be included in the initial credentialing packets. State criminal history background checks will also be conducted, the results of which will also be included in the initial credentialing packet. 2. With the written approval of the 55 WG/HC Security Manager, contract personnel may provide contract services prior to completion of background checks. However, at all times while children are in the care of that individual, the contract personnel shall be within sight and continuous supervision of a staff person, whose background check has been completed, a chaperone, or parent/guardian. 3. Contract employees shall have the right to obtain a copy of any background check pertaining to them and to challenge the accuracy and completeness of the information contained in the report. 4. Contract employees who have previously received a background check shall provide to the QAP proof of the check or obtain a new one.
