Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicit ation is issued as a Request for Proposal. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-03. Acquisition is100% set aside for small business concerns. The NAICS Code is 3 13311 and the small business size standard is 1000 Employees. The Offer Schedule (contract line item number(s), items, quantities, and units of measure) and Statement of Work are provided at the end of this document. Service is required to start on or ab out 15 Setember 2005. The provision at 52.212-1 Instructions to OfferorsCommercial Items, is applicable and is addended as follows: Paragraphs (b)(4), (b)(5), (b)(10), (d), (e), and (h) are deleted. This provision is further addended to incorporate 52. 252-1 Solicitation Provisions Incorporated by Reference (Fill-in information - www.gsa.gov); 52.216-1 Type of Contract (Fill-in information, firm fixed-price requirements); 252.204-7001 Commercial and Government Entity (CAGE) Code Reporting; and 252.209- 7001 Disclosure of Ownership or Control by the Government of a Terrorist Country. The provision at 52.212-2 EvaluationCommercial Items is applicable; Technical (Management Experience with similar service requirements) and Price are the evaluation criteri a in Paragraph (a). Technical experience is slightly more important than Price. The Government intends to make a single award to the responsive and responsible offeror whose offer is most advantageous to the Government considering price and non-price rel ated factors. The successful offero must meet the following requirements: a) Contractor must have an EPA Registered Treatment Product and must have all applicable Federal, State, and local licenses, registrations, and certifications. b) Contractor must have previous experience (two years or 300,000 uniforms) treating uniforms with Permethrin insect repellant with Department of Defense organizations. Offerors are instructed to submit completed copies of FAR Provision 52.212-3, Offeror Representations and CertificationsCommercial Items, and DFARS Provision 252.212-7000 Offeror Representations and CertificationsCommercial Items, with offers. Offerors should also submit their management experience (brief description of services provided, dollar amount and length of contracts or agreements, points of contact and telephone numbers and/or e-mail addresses) with similar services. See Numbered Note #1 OFFER SCHEDULE PERIOD OF PERFORMANCE: 15 Sept 2005 THROUGH 20 Oct 2005: 01 PERMETHRIN Uniform Treatment of the Army Combat Uniform (ACU). Each uniform set will include one uniform blouse and one uniform trousers. Approximate number of uniforms is 18,000. Payment will be made for actual uniforms treated, and returned in prop er condition. 02 TRANSPORTATION OF UNIFORMS FROM Ft. Campbell to treatment facility. The vendor will pick up approximately one half of the uniforms on/about 20 September. The second half of the uniforms will be picked up on/about 5 October. Delivery of treated unifor ms back to Ft. Campbell will be no later than 15 days after pick-up by the vendor. Payment will be made for two (2) pick-up/delivery dates regardless of the number of uniforms treated. STATEMENT OF WORK Concept of the Operation: The requirement is for Permethrin Insect Repellant Treatment for approximately 18,000 Army Combat Uniforms (ACUs). Two pick-up times will be scheduled at Ft. Campbell. The vendor will have a company representative at Ft. Campbe ll the day before each of the two (2) pick-ups scheduled to supervise the turn-in and pack-up process. The successful offeror will assist the unit representative with planning details to ensure an efficient pick-up and delivery of these uniforms. It is c ritical that the uniform a soldier turns in is the same uniform returned to him. The vendor will provide enough boxes so that each company sized unit ( ) can pack and turn-in their uniforms seperately. Pins/labels/tags/markers and all other uniform tur n-in necessities will be provided by the vendor. a. Vendor will treat each uniform set with Permethrin Insect Repellant. b. Vendor will provide transportation for uniforms between Ft. Campbell and the vendors treatment facility. Two (2) pick-up and delivery days will be scheduled to ensure all uniforms are turned in and treated. The return delivery day of the first load o f treated uniforms will a lso be the pick-up date for the second set of uniforms. c. Vendor will provide ID tags and pins to label each soldiers uniforms. d. Vendor will provide boxes and labels so that each unit can submit uniforms in a box and receive the treated uniforms back in the same container and condition. e. Vendor will be available to consult with the 4th BCT Unit Representative after award and before the Period of Performance (POP) begins to ensure all details are coordinated.
