Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation subpart 12.6, as supplemented with additional information included in this notice and following the simplified acquisition procedures in FAR part 13.5. This announcement constitutes the only solicitation; proposals are being requested. Solicitation number APVSNVXX-0203-5 is being issued as a request for proposal (RFP). The incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-4. The North American Industry Classification System (NAICS) code is 541710 and the Size Standard is 500 employees. The requirement is solicited on an open market basis for potential set-aside to a small business. If two or more small businesses firms submit responsive proposals the requirement will be set-aside to small businesses and award be made to that small business offering the lowest price. Should fewer than two small businesses submit responsive proposals, the requirement will be deemed unrestricted and award will be made based on the evaluation factors noted below. You are invited to submit a brand name or equal proposal for the supplies and services as listed in the attachment entitled ?LIST OF SUPPLIES AND SERVICES?. INTRODUCTION: USDA?s Animal & Plant Health Inspection Service (APHIS), Veterinary Services (VS) needs an Animal Resources Management (Medical Research) application with an integrated veterinary management system. The process covered starts with the submission of a research protocol. The protocol is distributed to the Animal Care and Use Committee members for review and comment. Following approval of the research protocol by the Animal Care and Use Committee, researchers can request the purchase of animals. Animal care staff will record the animal inventory with a bar code system. Researchers and veterinarians will be able to record information on the individual animals and access it either from the current network or from a remote location via a peripheral device to be supplied with this application. The proposed software application will have the ability to track the complete history of an individual animal, from approval of the research protocol, to arrival, to treatment of the animal, with the ability to access that record in the barn. Currently, a paper record is started for each instance of illness. Later, veterinarians and researchers must physically go to the previous paper records on that animal to look at prior diagnoses and treatments. An integrated veterinary management system will help to centralize the individual records for the animals in our various facilities. The application will help to capture the diagnosis and treatment information at the location of the animals in real time, and will also allow several different veterinarians to access the complete record for an individual animal, as well as groups of animals. The proposed software application must be 21 CFR Part 11 compliant, and GLP, GALP and GCP compliant. The proposed application would use an Oracle or SQL database, and allow for customization. Licenses, maintenance & support, training, peripherals, implementation support and project management are also required, as listed in the ?LIST OF SUPPLIES AND SERVICES?. Incorporated in full text: FAR 52.204-8?Annual Representations and Certifications (Jan 2005) (a) (1) If the clause at 52.204-7, Central Contractor Registration, is included in this solicitation, paragraph (b) of this provision applies. (2) If the clause at 52.204-7 is not included in this solicitation, and the offeror is currently registered in CCR, and has completed the ORCA electronically, the offeror may choose to use paragraph (b) of this provision instead of completing the corresponding individual representations and certification in the solicitation. The offeror shall indicate which option applies by checking one of the following boxes: [ ] (i) Paragraph (b) applies. [ ] (ii) Paragraph (b) does not apply and the offeror has completed the individual representations and certifications in the solicitation. After reviewing the ORCA database information, the offeror verifies by submission of the offer that the representations and certifications currently posted electronically have been entered or updated within the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard applicable to the NAICS code referenced for this solicitation), as of the date of this offer and are incorporated in this offer by reference (see FAR 4.1201); except for the changes identified below [offeror to insert changes, identifying change by clause number, title, date]. These amended representation(s) and/or certification(s) are also incorporated in this offer and are current, accurate, and complete as of the date of this offer. FAR Clause Title Date Change Any changes provided by the offeror are applicable to this solicitation only, and do not result in an update to the representations and certifications posted on ORCA. (End of Provision) PLACE OF PERFORMANCE: USDA, APHIS, VS, NVSL,1800 Dayton Ave, Ames, IA 50010 PERIOD OF PERFORMANCE: The period of performance for this purchase/task order will be 12 months from the date of award. GOVERNMENT ACCEPTANCE: The government shall have 30 days from the receipt of the last expected deliverable on to test and evaluate products, and to request fixes to software bugs and miscoded requirements found during this evaluation period. The contractor is expected to fix these problems within the scope of this firm fixed price contract. GOVERNMENT FURNISHED PROPERTY AND ITEMS ACQUIRED UNDER THIS CONTRACT: All equipment, documentation, reports and materials delivered under this contract will become the property of USDA Veterinary Services. INFORMATION TO BE INCLUDED WITH PROPOSAL: Each vendor is urged to examine this solicitation in its entirety and to ensure that their proposal contains all necessary information as follows: Technical proposal, past performance information, cost proposal, proprietary disclosure statement, key personnel resumes, relevant GSA Schedule information, if applicable, prompt payment terms, CAGE Code, TIN, DUNs number, and any other terms and/or conditions. EVALUATION FACTOR INFORMATION TO BE INCLUDED: Technical/Functional Capability. See the Software Application Matrix requirement listed in the ?LIST OF SUPPLIES AND SERVICES??Licenses. The integrated software proposed must meet ALL of the requirements listed. The proposed software application must be 21 CFR Part 11 compliant, and GLP, GALP and GCP compliant. The proposed application would use an Oracle or SQL database, and allow for customization. The prospective offeror must also demonstrate specialized expertise and experience in integrating applications in an existing Oracle-based infrastructure. The contractor shall possess the experience, expertise, and ability to conduct on-site training, implementation support, and project management. Contractors shall provide resumes for all proposed key personnel. Resumes shall provide sufficient detail to demonstrate the skills, experience, and qualifications required to perform the effort. (NOTE: If any changes in proposed key personnel should occur during actual performance, the Contractors shall submit the resume of the proposed replacement to the Contracting Officer?s Representative for approval). Past Performance. Contractors shall provide examples of prior, recent and relevant experience within the past 5 years for the same or similar services as required by this Request for Proposal. Past performance information shall include contract, points of contact with current telephone numbers, fax numbers, and other relevant information. The Government may also obtain past performance information from sources other than those provided by the Contractor. Price. A Firm Fixed Priced Purchase/Delivery Order will be issued for this effort. Your price quote must contain sufficient information for the evaluator to justify quantities and amounts. (1) Licenses ? as listed in the ?LIST OF SUPPLIES AND SERVICES? (2) Annual Maintenance and Support - as listed in the ?LIST OF SUPPLIES AND SERVICES? (3) Training - as listed in the ?LIST OF SUPPLIES AND SERVICES? (4) Servers - as listed in the ?LIST OF SUPPLIES AND SERVICES? (5) Barcode Scanners & Peripherals - as listed in the ?LIST OF SUPPLIES AND SERVICES? (6) Oracle License - as listed in the ?LIST OF SUPPLIES AND SERVICES? (7) Implementation Support and Project Management - as listed in the ?LIST OF SUPPLIES AND SERVICES? Travel Costs. Some travel will be required in order to accommodate Training, as well as, Implementation Support, as listed in the ?LIST OF SUPPLIES AND SERVICES?. The contractor will be responsible for all travel arrangements for contractor personnel. Travel will be reimbursed at cost, in accordance with FAR 31.205-46 ?Travel Costs?, and the Joint Travel Regulation (JTR). Other Direct Costs (ODC). Identify any other cost(s), not listed above, necessary to meet the requirements of the effort. BASIS OF AWARD: The Government will award a purchase/delivery order to the responsible offeror whose proposal conforms to the solicitation, and is considered to be the most advantageous to the Government, Price and other factors considered. The Government shall use the following criteria when evaluating proposals for award: Technical/Functional Capability, Past Performance, and Price. Technical/Functional Capability is the most important evaluation factor. Past Performance is somewhat less important than Technical/functional capability. Price is the least important evaluation factor.
