Description
SUBJECT: MEDICAL MONITORING AND SURVEILLANCE PROGRAM This is prepared in accordance with the format in FAR Part 13, Simplified Acquisitions, as supplemented with additional information included in this notice. QUOTES ARE BEING REQUESTED. Responses shall reference Request for Quotations No. DE-RQ26-05NT500846 the U.S. Department of Energy, National Energy Technology Laboratory (NETL), Pittsburgh, PA intends to purchase the following which is to be provided at our site THE ALBANY RESEARCH CENTER, 1450 QUEEN AVE SW, ALBANY, OR 97321: Occupational Medicine and Wellness Support for the Albany Research Center WORK STATEMENT September 15, 2005 Note: DOE/ARC personnel develop, interpret, set policy, and manage the occupational medicine and wellness programs for the ARC site. Description of Agreement: The vendor shall furnish supplies or services, as described below, if and when requested by the Contracting Officer (or an authorized representative) from the above effective date through 15 September 2006. The Government may renew the contract each year there after for a total contract term of five years depending upon the availability of funding. Call Orders will be made for the specific medical services by designated and authorized Government Personnel. Prices to be paid for individual requests, beyond the scope of this work statement and not otherwise covered by this agreement, will be established when calls are made. WORK STATEMENT. General Program. " The contractor shall implement and maintain programs that support ARC's occupational medicine compliance, monitoring, and surveillance requirements. " The contractor shall implement and maintain ARC's wellness initiative. " The contractor shall support the process of reviewing and revising ARC's occupational medical and wellness directives (e.g., orders, operating plans, and procedures). The contractor shall recommend updates to existing DOE and ARC plans related to these programs as deemed necessary. " The contractor shall provide recurring occupational medical services to ARC personnel as requested. Specific Programs. Occupational Medicine Program Support The contractor shall support the implementation and maintenance of an Occupational Medicine Program that is compliant with DOE, federal, state, and local requirements. This activity includes occupational medicine and health program support to ARC, including but not limited to the following elements: Administrative " Providing licensed physicians, registered nurses, and support personnel qualified to administer required deliverables. Point of delivery must be within a reasonable distance (less than 3 miles) of the ARC site to minimize travel time and provide easy access to all employees. Hours of operation and availability shall be consistent with normal business hours (falling within the range of 7:00 am - 6:00 pm). " Developing a customer satisfaction survey to be issued to patients randomly in which results shall be reported annually to the ARC Environment, Health, and Safety (ES&H) Manager. " Maintain employee records in confidence according to appropriate Government regulations - FAR 52.224-1 Privacy Act Notification and FAR 52.224-2 Privacy Act (provided). " Maintaining medical equipment and provide access to maintenance records for audit purposes.Provide all necessary equipment and supplies to perform the work. Emergency Medical " Providing first-aid and emergency care services at providers site as required. " Providing medical referral service when required. Emergency Response " Providing personnel to participate in the ARC's emergency response program as emergency medical advisors / responders and as official providers of patient medical history if an employee must be transported. " Providing fitness assessments for employees entering positions within the emergency response organization for which medical surveillance is a regulatory requirement. Medical Surveillance The provider shall include a Privacy Act notification on any form used to collect information on ARC employees. Wording shall be consistent with the following: "Privacy Act Statement Privacy Act of 1974, Public Law (P.L. 93-579) Collection of this information, to be contained in DOE Systems of Records DOE-33, is authorized by the Department of Energy Organization Act of 1977, P.L. 95-91. The primary purpose of the collection is to provide background information for use by medical personnel prior to routine medical examination. Relevant information may also be released to emergency personnel responding to an on-site medical emergency involving the individual who provided the information. Additional uses are established in DOE-33. Disclosure is voluntary for personnel in positions not subject to medical or physical suitability requirements. For personnel subject to medical or physical suitability requirements, 5 CFR Part 339 provides that refusal to submit to examination may result in appropriate disciplinary or adverse action." " Performing mandatory comprehensive baseline and follow-up (annual or as required) medical examinations and counseling. This service shall be applied to all ARC employees (e.g., employees having a Federal identification badge) whose work and potential job-place exposures mandate their inclusion in the ARC medical surveillance-monitoring program. It may also apply to certain permanent contract employees, as identified by authorized ARC personnel, in certain situations such as blood draws for Beryllium Lymphocyte Proliferation Tests (BeLPT), as required. The contractor shall plan on conducting a maximum of 100 physical examinations under ARC's mandatory medical surveillance program during any fiscal year. These examinations shall follow and meet all requirements set forth in DOE Order 440.1A (Review Date 03-27-00) and DOE Final Rule, 10 Code of Federal Regulations, Part 850, Chronic Beryllium Disease Prevention Program (December 8, 1999). These examinations shall include Complete Blood Count (CBC), Beryllium Lymphocyte Proliferation Test (BeLPT) - Blood (Submitted to National Jewish Medical and Research Center for analysis) as required, Blood Chemistry (CHEM), Electrocardiogram (EKG), Hearing (Audio Function), Hepatitis (HEPA), Pulmonary Function Test (PFT), Graded Exercise Stress Test (STR), Urinalysis (URN), Vision, and Chest X-Ray. These examinations shall also include the offering of PSA tests for men and PAP smears for women. Additional job-specific tests may be required and will be negotiated and agreed to by the ARC Environment, Health, and Safety (ES&H) Manager and the provider. Recommendations from the provider based on baseline and developing data will be considered. " Post Exposure Exams and Medical Surveillance for Asbestos, Lead, Radiation, Beryllium, Copper Oxide (Electricians), Chemical Sensitization, and Heavy Metals as required. " Medical Clearances for Respirator use [supplied air (e.g. sandblasting), negative pressure full face, ??-face, and dust mask] and restricted duty including working at heights, confined space, manual lifting, and chemical intolerance. " Fitness for Duty including Return to Work Clearance for both on- and off-job illness/accident as required, and Continuation of Work for pregnancy (F), reproductive (M or F), and/or Allergy (sensitivity) as required. " Maintaining the hearing conservation program to include administering the audiometric testing, evaluations, referrals, etc. with summary reports to the ARC ES&H Manager on problematic cases on case- by- case basis and annual summary report on all ARC staff.. " Providing a link with industrial hygiene services to integrate industrial hygiene monitoring results with medical care. " Providing professional consultation to the ergonomics program evaluators and employees with occupational cumulative trauma. " Providing medical surveillance through monitoring job tasks, physical evaluations, and case management for occupational injuries/illnesses. " Providing fitness assessments for employees whose jobs require medical surveillance or whose jobs will require physical exertion, such as respirator use or climbing, and/or foreign country travel. " Closeout examinations for employee terminations/retirements. Prevention " Providing and administering immunizations and other injections. " Providing flu shots annually. " Conducting regular (i.e., at least 4 per year) worksite training visits to educate employees on work-related or work site health risks. Topics will be mutually agreed to in advance through discussions with the ARC ES&H Manager. Training Providing training support to industrial hygiene personnel for programs such as hearing conservation, ergonomics and other agreed to topics. " Providing coordinating support for medical-related training including CPR, First-Aid, and AED. Wellness Program Support The contractor shall support the implementation and maintenance of a Wellness Program that is compliant with DOE, federal, state, and local initiatives. This activity includes wellness program support to ARC, including but not limited to the following elements: " Providing wellness educational programs that support the mental and physical well-being of ARC employees, including but not limited to the following elements: smoking cessation, general health, men's vs. women's health topics, exercise and nutrition advice, preventive health measures, cancer awareness, seat belt safety, first-aid, CPR, home safety initiatives (poison prevention, coping with stress, and other agreed to topics). Option to Extend the Term of the Contract (a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years. A FIXED PRICE PURCHASE ORDER WILL BE AWARDED FOR ONE YEAR AND 4 OPTION YEARS. WILL NEED FIXED PRICING FOR THE FOLLOWING: BASE YEAR - __________________ OPTION YEAR 1 - _______________ OPTION YEAR 2 - _______________ OPTION YEAR 3 - _______________ OPTION YEAR 4 - _______________ THE FOLLOWING PROVISION IS INCLUDED IN THIS RFQ: FAR 52.217-5 EVALUATION OF OPTIONS EXCEPT WHEN IT IS DETERMINED IN ACCORDANCE WITH FAR 17.206(b) NOT TO BE IN THE GOVERNMENT'S BEST INTERESTS, THE GOVERNMENT WILL EVALUATE OFFERS FOR AWARD PURPOSES BY ADDING THE TOTAL PRICE FOR ALL OPTIONS TO THE TOTAL PRICE FOR THE BASIC REQUIREMENT. EVALUATION OF OPTIONS WILL NOT OBLIGATE THE GOVERNMENT TO EXERCISE THE OPTION(S). DELIVERY: FOB Destination is required (all freight included). The provisions at 52.212-2 Evaluation ??? Commercial Items (JAN 1999) does not apply to this acquisition. Instead, the following information will be used for evaluation of offerors: An award shall be made to the responsible offeror submitting an acceptable quote and offering the lowest evaluated price. Evaluation is based on best value including cost and ability to meet stated requirements above. Offerors shall submit descriptive literature and drawings detailing features (if required), technical capabilities and warranty data. Acceptability will be determined solely on the content and merit of the information submitted response to this provision as it compares to the minimum characteristics provided above. Therefore, it is essential that offerors provide sufficient literature, documentation, etc., in order for the Government evaluation team to make an adequate technical assessment of the quote as meeting acceptability. Price shall be the deciding factor among acceptable quotes. The North American Industry Classification (NIAC) is 621511. ALL INTERESTED PARTIES SHALL SUBMIT OFFERS WITH THE FOLLOWING INFORMATION: Federal Tax Identification (TIN); Dun & Bradstreet Number (DUNS); and remit to address if different. A Firm Fixed Priced Purchase Order shall be issued using the Simplified Acquisition Procedures FAR Part 13. Responses/offers are due no later than 5:30 p.m. Eastern Time Zone on September 22, 2005.
