Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This solicitation is 100% set-aside for Small Business under North American Indust ry Classification System Code 336321, with a small business size standard of 500 employees. (A) SPECIFICATIONS: The Utah National Guard intends to purchase and have installed Whelen or equal emergency lights and sirens for the following vehicles: (1) 2 ea 2004 ? ton Chevrolet dually pickups with utility shell; (2) 1 ea 2004 Chevrolet Suburban; (3) 1 ea 2004 Chevrolet Suburban with satellite dish on roof; (4) 1 ea 2004 ? ton Chevrolet pickup with utility shell; (5) 1 ea 2004 Chevrolet cargo van (1 ton chassi s); (6) 2 ea 2004 GMC C6500 with shelter; (7) 1 ea 20 Wells Cargo trailer; (8) 2 ea 19 6 trailer and; (9) 1 ea 19 6 Haulmark trailer. Offerors are urged to attend an organized site visit to inspect the vehicles to be conducted on September 13, 2005 a t 1:00 P.M. (B) APPLICABLE PROVISIONS AND CLAUSES: The foll owing clauses and provisions are incorporated either by reference or full text and are to remain in full force in any resultant purchase order: (1) FAR 52.212-1 Instructions to Offerors--Commercial Items (JAN 2005), Addendum paragraph (h) Multiple Awards  delete in its entirety, the government plans to award a single contract resulting from this solicitation; (2) FAR 52.212-2, Evaluation--Commercial Items (JAN 1999); (3) FAR 52.212-3, Offeror Representations and Certifications--Commercial Items (MAR 2005) ; (4) FAR 52.212-4 Contract Terms and Conditions--Commercial Items (OCT 2003); (5) FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders  Commercial Items (JUN 2003); (6) FAR 52.222-21 Prohibition of Segregated Faci lities (FEB 1999); (7) FAR 52.222-26 Equal Opportunity (APR 2002); (8) FAR 52.222-35 Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and other Eligible Veterans (DEC 2001); (9) FAR 52.222-36 Affirmative Action for Workers with Disabilities (JUN 1998); (10) FAR 52.222-37, Employment Reports on Special disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans (DEC 2001); (11) FAR 52.232-33 Payment by Electric Funds Transfer-Central Contractor Registration ( OCT 2003); and (12) DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items (JUN 2005). (C) EVALUATION FACTORS: The contract type for this procurement will be fix ed price and award will be made on the basis of best value to the government, price and other factors considered. Selection will be based on the following evaluation factors, in order of importance: (1) technical knowledge of this type of equipment, offer ors capability to provide the items required, and the quality of the proposed item; (2) past performance, caliber of offerors performance on previous contracts; and (3) total cost or price. (D) INSTRUCTIONS: Offers shall include (1) a cost proposal, incl ude descriptive literature of products being proposed; (2) FAR 52.212-3 Representations and Certifications  Commercial Items, filled in completely; (3) a list of at least three references whose contract has been completed within the past three years or cu rrently in process of similar work covered by this solicitation. Provide the following information for each reference: (a) contract number; (b) name and addres s of contracting activity; (c) contracting officers name and telephone number; (d) period of performance of contract; (e) brief description of work under the contract. Failure to provide items 1-3 listed above with your offer, may cause your offer to be considered non-responsive. To receive award, the offeror must be registered in the Central Contractor Registration (CCR) database, which can be accessed at www.ccr.gov. Offers shall be received NLT 2:00 P.M.
