- The purpose of this amendment is to change the date the quotes are due to September 21, 2005 10:00 a.m. central daylight time (cdt) which is shown as the Response Date. A MANDATORY SITE VISIT IS REQUIRED AND CAN BE SET-UP BY CALLING DENISE HENANDEZ 305-526-2926 EXT 230 OR LISA ROBICHEAU 305-526-2926 EXT 222. --------------------------------------------------------------------------- The United States Department of Agriculture (USDA), Animal and Plant Health Inspection Service (APHIS), and Veterinary Services (VS), 6300 NW 36th Street, Miami, FL 33122 has a requirement for 20 - 24 temporary/portable equine stalls. (i) THIS IS A COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is APVSERGV-0014-5 and the solicitation is issued as a request for quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulation (FAR) and Agriculture Federal Acquisition Regulations supplement (AGARs) can be accessed on the Internet at http://www.arnet.gov/far and http://www.usda.gov/procurement/policy/agar.html. (iv) This solicitation is a small business set-aside. The associated North American Industry Classification System (NAICS) code is 423390; business size standard for this code is 100 employees. (v) (vi) This requirement consists of 1 line item to design a temporary/portable stall facility approximately 72' X 45' (construction diagrame sample shown in attachment ?Drawing (Stalls).pdf?) with special attention concerning marked posts and hallway entrances. Front & door panels should be half grill sliding door or similar with plastic wood or similar product. Side & back are full panels of plastic wood or similar product. It is anticipated that a firm-fixed price contract will be awarded. The Government intends to issue a single solicitation and make a single award to one offeror. Simplified procurement procedures will be used as well as commercial item acquisition regulations per FAR 12. The solicitation and all subsequent notices and amendments will be posted on the Internet at: http://www.fedbizops.gov. Before contractor is chosen, manufacturer must meet with Dr. Richard Cambre at the program to approve layout. (vii) Delivery and acceptance of line item will be FOB destination, Miami, FL. (viii) The provision at 52.212-1, Instructions to Offerors ? Commercial Items, applies to this solicitation. (ix) The provision at 52.212-2, Evaluation ? Commercial Items, applies to this solicitation. The following text is added to Paragraph (a) of FAR 52.212-2: The following factors shall be used to evaluate offers: Technical capability of the item offered to meet the Government requirement; Past performance; Price. Technical and past performance when combined, are approximately equal to price. ALL RESPONSIBLE SOURCES MAY SUBMIT A QUOTATION FOR CONSIDERATION. All offers must be signed. Offers must include tax payer federal identification number, DUNS number, and business size. Failure to provide this information will result in vendor not being considered for award. Effective October 1, 2003, the Federal Acquisition Regulation (FAR), requires that contractors be registered in the Central Contractors Registration (CCR) System before they can be awarded contracts. A mandatory site visit is scheduled for July12, 2005, call Nils Forseth at 305-526-2926 ext 255 to schedule an appointment during 10:00 a.m est. ? 2:00 p.m. est. Nils Forseth (Quarantine Enforcement Officer) will be the technical point of contact during the site visit. The USDA APHIS Veterinary Services is located at 6300 NW 36 street. We are on Perimeter road which runs parallel to NW 36th Street. Driving from I-95 North or South exit on 112 West airports. Continue straight and then exit on LeJeune Road and NW 36th street, turn left on 57th avenue and turn right immediately into Perimeter Road. Pass the light on 62nd avenue and follow the signs to the USDA Animal Import and Export Center. Driving from the Palmetto Expressway exit on North West 36th street (towards the airport) east go straight and turn right on 67 Avenue and turn left immediately towards Perimeter Road. Follow the signs to the USDA Animal Import and Export Center. Driving from the turnpike exit on (41 street which is also Doral) you will need to go east towards the airport. You will make a right on 67 avenue and immediately make a left at the next light which is Perimeter Road. Continue on perimeter road and pass the UPS entrance on your right. Make a right on the next entrance. Our building is on the left hand side. If you are at the 62 avenue light you have passed our facility.
