Description
FA6703-05-Q-0096. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA6703-05-Q-0096 is hereby issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-05, effective 27 July 2005 and Air Force Acquisition Circular (AFAC) 2005-0818, effective 18 August 2005. The procurement is 100% small business set-aside and will follow simplified acquisition procedures in FAR Part 13. The applicable North American Industry Classification Code (NAICS) is 811412 with a size standard of $6.0M. Dobbins Air Reserve Base, (Marietta) Georgia requires the following nonpersonal services: The contractor shall provide all labor, tools, equipment, transportation, materials, and other services required for the cleaning, maintenance and repair of 14 each electric washers and dryers located in 5 different locations on Dobbins Air Reserve Base (DARB), (Marietta,) Georgia. The contractor shall be responsible for monthly inspection and cleaning of all dryer vents and shall insure they are firmly attached to avoid coming loose. The contractor shall monitor and inspect all washers monthly and ensure the equipment is operational and performing per the maunufacturers requirements. The contractor shall submit a monthly inspection schedule to the Government Quality Assurance Evaluator (QAE) for all washers and dryers. All prospective offerors are advised that these washers and dryers will receive a very high usage, above the normal wear and tear, due to training missions conducted on a monthly basis. Frequent replacement of control knobs, lint screens, timers, screws, agitators, transmissions, etc. shall be considered in the submitted offer. Prior to performing any maintenance the contractor shall report any suspected abuse to the QAE. The contractor will prepare and submit Service Tickets for all service calls received. All equipment shall be operational within 48 hours of the contractor being notified of a service call. Should repair of the equipment exceed 48 hours the contractor must notify the QAE immediately upon this determination being made. The contractor Repair Technician shall be available during the hours of 7:30 AM to 4:30 PM, Monday through Friday. Prospective offerors shall submit quotes as follows: CLIN 0001: Inspect and Maintain 14 each Washers/Dryers @ $XXX per month for 12 Months equals $XXXX per year: CLIN 0002: Out of cycle Service Calls Estimated 24 each @ $XXX per Service Call; CLIN 0003 Reimburseable Repair Parts/Supplies @ $XXX Lump Sum (Estimated Yearly Cost). The contractor shall ensure the protection of all associated government property during the performance of the contract. Period of performance will be for one basic 12 month period with four (4)-12 month additional option periods. Pricing for each option period must be submitted in conjunction with pricing for the basic year, i.e., Option Period One, CLIN 1001, 1002, 1003, Option Period Two, CLIN 2001, 2002, 2003, etc.. One pre-bid site visit will be conducted on Thursday, 15 September 2005 at 1:00 P.M., at building 812, Suite 106, 1538 Atlantic Avenue, Dobbins ARB, GA 30069-4824. Attendance is not mandatory, however, failure to attend will not relieve bidders/offerors from responsibility for estimating properly the cost of successfully performing the work. This will be the only site visit conducted to familiarize the offerors with the project location. All interested parties should notify the Base Contracting Office, 94 LG/LGC, by telefax at 678/655-5612 no later than 12:00 P.M., Wednesday, 14 September 2005 of their intention to attend the site visit. The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors--Commercial Items, shall apply along with FAR Clause 52.204-7, Central Contractor Registration (CCR). All prospective offerors must be registered in the Central Contractor Registration (CCR) database prior to submission of an offer to be considered for award of any Department of Defense (DoD) contract. This must be accomplished electronically at: http://www.bpn.com. Evaluation of quotes will be in accordance with FAR Clause 52.212-2, Evaluation-Commercial Items and will be based on price only. All prospective offerors shall complete FAR Clause 52.204-8, Annual Representations and Certifications (JAN 2005)-Commercial Items (This requirement must be completed electronically on-line at http://orca.bpn.gov ). FAR Clause 52.212-4, Contract Terms and Conditions--Commercial Items, shall apply. FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, shall be incorporated into any contract resulting from this notice. Applicable clauses cited in 52.212-5 are 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-41, 52.222-44, 52.225-3 (Alt I), 52.225-13, and 52.232-33. Defense Federal Acquisition Regulation Supplement (DFARS) Clause 252.232-7001, Disclosure of Ownership or Control by the Government of a Terrorist Country, 252-212-7000, Offeror Representations and Certifications?Commercial Items, 252-212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items shall also apply. FAR Clause 52.228-5, Insurance--Work on a Government Installation shall apply and will be incorporated into any resultant contract. Department of Labor Wage Determination 94-2133 (Rev 28), dated 05/23/2005 shall apply and be incorporated into the resulting contract.
