Description
CLASSIFICATION CODE: G SOCIAL SERVICES This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This is a 100% Total Small Business Set-Aside; all responsible sources may submit a quote. The applicable North American Industry Classification System Code (NAICS) is 813110 and the size standard is $6 million. The Government intends to issue a pre-priced blanket purchase agreement (BPA) for Auxiliary Catholic Priest Services at McGuire AFB, NJ. Delivery will be made to McGuire AFB, New Jersey on an as needed basis indefinitely. Method of payment will be Electronic Funds Transfer. All responsible sources may submit a quote, which will be considered by the agency. All prospective offerors interested in submitting a quote on this solicitation must have a Commercial Government Entity Code (CAGE Code). All potential sources must obtain a DUNS number and register in the Central Contractor Register (CCR) in order to transact business with the government. Failure to obtain a DUNS number, register in ORCA, and register in the CCR may result in the delay of award of a contract or possible award to the next otherwise successful offeror who is registered at the time of award. One of the following paragraphs will apply: (a) Prospective contractors shall complete electronic annual representations and certifications at http://orca.bpn.gov in conjunction with required registration in the Central Contractor Registration (CCR) database (see FAR 4.1102). or (b) Prospective contractors shall update the representations and certifications submitted to ORCA as necessary, but at least annually, to ensure they are kept current, accurate, and complete. The representations and certifications are effective until one year from date of submission or update to ORCA. Note: A search in ORCA will be accomplished to ensure compliance. Services will be performed in support of the McGuire Air Force Base Chapel Program. 1. The services stipulated in this Statement of Work shall be performed at the Base Chapel, 2503 E. Arnold Avenue, McGuire AB NJ 08641, or within the confines of McGuire AFB. 2. The U.S. Government will not supervise the contractor in any manner. 3. Qualifications: a. As stipulated by the Armed Forces Chaplainry Board of the Department of Defense, the contractor will retain required ecclesiastical faculties and certification required by the proper Catholic endorsing agent. b. By interview with Catholic Chaplain, contractor must be able to demonstrate the necessary skills, knowledge and willingness necessary to work in the military environment. 4. Duties/Responsibilities. The contractor will: a. Coordinate, organize and prepare for duties as the Auxiliary Chaplain. b. Meet with Wing Chaplain and/or Catholic Chaplain monthly to plan schedule of work. c. Coordinate with Wing Chaplain and or Catholic Chaplain concerning special services or coverage of a service due to the absence of the active duty priest/chaplain. d. Provide, when required, customary coverage for the following: Roman Catholic Liturgy, weddings, Baptisms, hospital ministry, sacramental preparation, annulments and emergency ministrations. e. Follow canonical requirements and established procedures and norms in use locally for liturgical and pastoral ministration. f. Notify the Wing Chaplain and/or NCOIC of Chapel Support Activities of any special support requirements for Catholic Services. g. Meet with Wing Chaplain and/or Catholic Chaplain periodically during the contract period to discuss/review the status of the Catholic program and exchange feedback. 5. Rate of Pay: Rate of pay will be monthly. 6. Contractor Performance: Performance under this contract will be evaluated by: a. Customer Feedback/Survey will be done to evaluate your work. b. Customer satisfaction from students and parents concerning the quality of service. c. Comments will be made to state ways of improvements. 7. Equipment/Supplies: a. Requirement for supplies (i.e. music, crafts, fellowship supplies, etc.) will be submitted in writing to the Sponsoring Faith Group Chaplain in accordance with local operating procedures. b. Government-owned equipment will not be tampered with or moved without permission of the Senior Chaplain. 8. Security of Facility: The contractor will ensure that all facilities are properly secured upon departure following the conclusion of a service/rehearsal when a Chaplain/Chaplain Assistant is not present. 9. Not engage in the unauthorized duplication of copyrighted materials by photocopying or any other means that would expose the government to the liability for penalties under existing or future copyright laws. 10. Security: All personnel under this contract must complete a positive background investigation. 11. Contractor will locate and arrange for a suitable replacement if Prime Contractor is not available for any required service(s). The contractor shall be responsible to ensure the replacement can perform subject services as called for in the statement of work. The substitute must meet or exceed the qualifications of the Prime Contractor and may be required to demonstrate capabilities to the Chapel staff. The contractor shall provide name(s) and social security number(s) of replacement(s) by written letter to the contracting officer to be made part of this contract. Any changes to the list must be resubmitted for contract inclusion. Providing for replacements shall not relieve the Prime Contractor from performing services under this contract. Early terminations will be processed in accordance with FAR Clause 52.212-4(m), Termination for Cause. The Prime Contractor shall be liable for all re-procurement costs. 12. Performance of services during crisis declared by the President, Secretary of Defense, or overseas combatant commander: Provide 24-hour contact, and comply with all provision of this Statement of Work during a crisis declared by the President, Secretary of Defense, or overseas combatant commander. Workloads may significantly increase during wartime contingencies as directed by the contracting office.
