Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Regulations document (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a proposal is requested, and a written solicitation will NOT be issued. The purchase request number is F2V4FM5237A600. The solicitation incorporates provisions and clauses as those in effect through Federal Acquisition Circular 01-05. DESCRIPTION: NAICS code is 532490 with the size standard of $6M. STATEMENT OF NEED General Description: The contractor shall provide a committed source of supply of medical-grade liquid bulk oxygen for the 6th Medical Group. Administration: - All services rendered will be itemized by date of service, product and quantities provided, unit price, and total amount of the invoice. Any discounts offered shall be apparent on the face of the invoice. Deliveries/Location and work hours: - Deliveries will be made Monday-Friday (excluding holidays, New Year?s Day, Martin Luther King Day, Presidents Day, memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, and Christmas Day) between the hours of 0730 AM to 1630 PM, except in the case of emergency. - Deliveries will be supervised by the COTR or their designee before beginning delivery operations. Contractor will contract the COTR prior to delivery to arrange for delivery supervision. - Deliveries will be made to: 6 Medical Group, Bldg 709 8415 Bayshore Blvd MacDill AFB, FL 33621 General: The quantities shown in the Statement of Work (SOW) schedule are estimates of the 6th Medical Group annual requirements. There is no express or implied guarantee that these quantities will be purchased. Please note that any resultant contract will include the installation of contractor-owned equipment will include a 90 day transition period at the beginning and end of the contract period. The contract will include one (1) base year and four (4) one year option renewal periods. Prior to first filling and annually, contractor must perform in-service training to 6 Medical Group staff, which will include the following facets for contractor owned and government owned systems: the refill procedure, any preventive maintenance support requirements that may be needed from the medical center systems, and an explanation of all the volume alarm and low pressure set-points. The contractor will provide written procedures and training for 6 Medical Group staff for protocols to accomplish emergency shutdowns or other sudden, unplanned termination of the refilling process. Contractor will provide 24/7 emergency contact name(s) and telephone number(s). Prior to first filling, and annually thereafter, a written verification must be presented using a qualified third party expert per NFPA 99, 2002 Edition for contractor owned and government owned systems. Any code deficiencies in the Medical Center?s existing system, as defined by NFPA 50, ?Bulk Oxygen System?, must be identified by the contractor. A detailed explanation of these deficiencies must be presented in writing to the COTR for concurring signature. Prior to the first filling, and semi-annually thereafter, the contractor must verify, in writing, the accuracy of all gauges on contractor owned tanks. Government-Furnished Property: The 6 Medical Group has an existing location and foundation for the installation of the contractor owned bulk oxygen tank(s). Additionally, the 6 Medical Group shall provide access to an electrical power source and hook-up to a facility-maintained alarm system. The contractor shall perform the hook-up of contractor owned equipment to the facility-maintained alarm system. The government will provide a point for interconnection of the contractor-owned system with the internal hospital distribution system. This interconnection point will be within the vicinity of the bulk liquid oxygen tank. The contractor will provide all necessary materials and labor to reach the point of interconnection. Contractor-Furnished Equipment: The contractor shall provide, install, and maintain bulk oxygen tank with appropriate back-up system. The tank capacity and reserve system shown in the schedule are approximate capacities required by the 6 Medical Group. The contractor will propose the appropriate sized main and reserve system. Based on historical use and delivery schedule, the COTR and contractor will agree upon the correct size system. Liquid converter (vaporizer) economizer, alarm sensors, regulator valves, level indicators, and any other devices or connections required for proper tie-ins with the facility?s gas system shall be furnished by the contractor, without cost to the Government. The liquid converter shall deliver gas at a pressure and rate of flow adequate to supply the system. Each liquid oxygen container shall have an outlet that allows access for testing the purity of the oxygen. Bulk liquid oxygen system will be installed and maintained in accordance with NFPA 50 and any other applicable codes or standards. All equipment and materials required to perform on the contract (other than what is specifically listed as Government-Furnished Property) shall be provided by the contractor. Contractor owned equipment shall be installed, inspected and maintained by the contractor without additional cost to the Government. (i.e. all installation, inspection and maintenance costs shall be included in the contract?s monthly equipment rental fee.) Contractor owned equipment shall be maintained in good operating condition in accordance with applicable regulations. Appearance of equipment will also be appropriately maintained, and tanks will be properly painted, free of rust and corrosion, etc. Tanks will be labeled according to any applicable FDA, DOT, OSHA, or other requirements. The contractor shall perform semi-annual preventive maintenance on the contractor-owned equipment. This maintenance will be performed as a minimum at six-month intervals or more often. Maintenance shall include all necessary inspections, adjustments, repairs and other services necessary to assure the system is in proper condition. During these periodic services, the contractor will verify proper alarm operation with 6th Medical Group staff. Copies of inspection reports and maintenance records will be provided to the COTR. The contractor will have a 24 hour/7day emergency maintenance and repair capability. Qualified maintenance personnel will respond on-site within 2 hours of notification. Through the duration of the contract, the contractor shall be liable for the integrity, suitability, and safety of contractor owned tank(s) and will insure compliance with applicable regulations, standards and normal good practices. Installation of Contractor-Owned Equipment: Unless otherwise directed by the 6th Medical Group, contractor owned equipment shall be installed and connected to the medical gas system on the date contract becomes effective; provided that the contractor shall be allowed at least thirty days after receipt of notice of award to complete installation. If the contractor?s equipment replaces equipment already in use, the exchange of equipment shall be accomplished without interruption of gas supply to the 6 Medical Group. A temporary, portable oxygen supply will be connected to the facility?s emergency oxygen supply connection if necessary. Contractor installed equipment shall remain the property of the contractor and shall be removed upon termination of the contract, when directed by the 6 Medical Group facility and in full cooperation with the succeeding contractor so as to avoid interruption of gas supply. To permit orderly transition from one contractor to another, the contractor shall continue to honor the contract?s monthly equipment rental fee and bulk oxygen contract price for a maximum of ninety days beyond the scheduled expiration of the contract period. The contractor shall continue to provide and maintain its equipment during this transition period. All contractor owned equipment shall be installed in accordance with NFPA 50: Standard for Bulk Oxygen Systems at Consumer Sites, (current edition), NFPA 99: Health Care Facilities (current edition) and FDA?s Current Good Manufacturing Practices (CGMP) Regulations. The contractor shall comply with all OSHA standards and applicable safety requirements, including proper signage and use of personal protective equipment. The contractor will coordinate with 6 Medical Group personnel the connection and testing of the bulk oxygen system?s alarm sensors to the hospital alarm panels. The contractor shall have a qualified third party certify the system for use at the time of final connection and prior to operation at the healthcare facility. A written copy of the certification report will be provided to the COTR. Delivery of Medical Liquid Bulk Oxygen Deliveries of oxygen are scheduled by the contractor based on information transmitted by the wireless oxygen level monitoring system. The contractor will have a delivery ?forecasting program that assesses the 6 Medical Group recent and historic usage and determines when replenishment of the liquid oxygen supply is needed. The reorder, alarm, safety, and reserve levels will be coordinated with the COTR. Alarm levels will be set at such points to avoid frequent alarms. The delivery system shall be designed so that scheduling of the replenishment of the oxygen supply requires no intervention on the part of 6 Medical Group personnel. The contractor shall deliver medical-grade liquid oxygen within 48 hours after the reorder point on the main tank is reached. If the 48 hours ends on weekend, delivery will be on Monday. If for any reason the contractor is unable to deliver at the agreed upon day or time, the contractor will provide 24-hour notice to the COTR, so that the facility can initiate an alternate backup action. In accordance with HI Patient Safety Alert dated March 22, 2005, all deliveries must be monitored by a qualified and trained technical representative (COTR) from 6 Medical Group. The contractor will be provided with names and contact information of primary and back-up facility representatives. This applies to all deliveries regardless of time or day of execution. The tank will be filled to maximum functional capacity at each refilling procedure unless otherwise specified in the facility requirements or as agreed upon in a written document signed and dated by the COTR. If the tank cannot be filled to maximum capacity at the time of delivery, the driver must annotate the reason for the partial delivery and indicate when a subsequent delivery will be made to completely fill the tank to capacity, at no additional cost. At the time of each delivery, contractor must provide a legible signed and dated written document that identifies the tank level prior to fill, the level after fill, and the quantity delivered. This document must be counter-signed by the facility representative supervising the delivery. The contractor shall have an emergency response plan that provides for emergency deliveries of oxygen to be delivered to the facility within two hours after notification, 24 hours a day, seven days a week, in accordance with the contract. In the case of a failure of the contractor?s bulk supply system at the 6 Medical Group, the contractor shall deliver a portable system, have it connected to the Emergency Oxygen Supply Connection, and have it operable with 2 hours of notification. The Emergency Response Plan must be submitted prior to contract award. Quality Assurance Specifications and Requirements a) Name of the Product (i.e. Oxygen U.S.P.) b) An Air Liquefaction Statement where appropriate c) Lot number or other unique identification number d) Actual analytical results for full U.S.P. monograph testing. (A statement that only proclaims that the product meets the minimum purity of 99.5%, etc. is not acceptable.) e) Test method used to perform the analysis. (A statement such as ?Meets U.S.P. specifications? is not acceptable; nor would ?Tested via Servomex? be acceptable since the specific model number is not provided.) f. Signature of authorized supplier representative and date. Material Safety Data Sheets shall be provided to the facility COTR at the beginning of the contract period and upon request. A copy of all inspection reports shall be provided to the facility COTR upon the completion of any bulk oxygen system inspections that are required by regulation. All contractor owned equipment shall be maintained or repaired in accordance with NFPA 50: Standard for Bulk Oxygen Systems at Consumer Sites, (current edition), NFPA 99: Health Care Facilities and FDA?s Current Good Manufacturing Practices (CGMP) Regulations. Security Requirements The contractor shall coordinate with the customer to ensure access to MacDill AFB. The contractor shall be responsible for obtaining a base entry in accordance with the base entry requirements and will be subject to background security check through the 6th Security Forces. This pass shall only be used to gain entry to the base to perform services as needed to fulfill contractual requirements. This pass shall not be used to gain entry to any other facilities/areas on base. Qualifications and Training If requested, contractor will provide records of training and competency assessment of all contractor personnel involved with the performance of this contract. Contractor will provide initial training and annual refresher training, at no extra cost, to all 6 Medical Group personnel involved with the delivery procedure and alarm monitoring of the liquid oxygen system. This training may need to be conducted at various times to accommodate different shifts. Applicable Regulations & Standards: The following is a list of some of the regulations and standards that are applicable to this contract. The list is not comprehensive, and the contractor is responsible for ensuring that all products, equipment and services provided under the contract are in compliance with applicable Federal State, and local regulations. If applicable, the editions in effect as of the date of this solicitation are listed. Contractor is responsible for remaining compliant with any future revisions that are effective at the time of contract performance. Title 21, Code of Federal Regulations, Parts 210 & 211 ? CGMP regulations for supplying medical-grade oxygen. (FDA) 29 CFR 1910.104. Applies to the installation of bulk oxygen systems on industrial and institutional consumer premises. (OSHA) 49 CFR - Transportation NFPA 50: Installation and Maintenance of Liquid Oxygen Systems at Customer Sites NFPA 99: Health Care Facilities Joint Commission on Accreditation of Healthcare Organizations: Environment of Care and other applicable standards.
