To include the reagents, supplies, delivery dates as scheduled on PR Web. Galileo Automated Analyzer Components: Instrument Platform Reader Computer Processor Interface capability Location: Laboratory / Blood Bank Coverage: 5 Days per week, Mon-Fri 8:00A.M.  5:00P.M. Technical service - 24 hour technical phone support 2. GENERAL SCOPE OF WORK 2.1. The contractor shall provide equipment on site, installation, and training. Provide all support needed to include equipment repairs (including reagent consumables), labor, and test equipment, travel, and materials not government furnished, necessary to keep the unit operational to manufacturers specifications, five (5) days per week. Provide troubleshooting assistance as requested 24 hours per day. 7 days a week. Shall provide and install all hardware and software updates. Shall provide new or upd ates user manuals when changes are made. New and refurbished parts must be certified and documented to meet OEM (Original Equipment Manufacturers) specifications on all manufacturers mandatory modifications and apply recalls by FDA. Provide a minimum of two (2) preventive maintenance services per year to include lubrications, cleaning (other than operators), calibrations, parts replacement, and unlimited service calls during the lease period. 2.2. PERSONNEL: 2.2.1. HOURS OF OPERATION: The instrument services shall be provided twenty-four hours a day seven days a week. 2.2.2. MINIMUM PERSONNEL QUALIFICATIONS: Services under this contract shall be performed by individuals who have demonstrated knowledge and competence in the servicing and maintaining of the specific instrumentation under the contract. The Contractor sha ll ensure repair technicians have been trained on the specific equipment under contract and have two (2) years of experience in maintaining equipment specified in number 1 above. 2.2.3. SECURITY REQUIREMENTS: Contractor personnel or any representative of the contractor entering Fort Benning shall abide by all security regulations and shall be subject to security checks. 2.2.4. PERSONAL APPEARANCE: Contractor personnel working under this contract shall practice high standards of personal hygiene and maintain a clean, neat appearance while on duty. 2.3. QUALITY ASSURANCE: Upon completion of work, an acceptance inspection will be conducted by the Government (personnel from Medical Maintenance) and equipment user to ensure all services are completed. 2.4. QUALITY CONTROL: The contractor shall be responsible to ensure that the quality of service and materials provided under this contract meet or exceed the performance work statement. 3. DEFINITIONS: 3.1. PREVENTIVE MAINTENANCE CHECKS AND SERVICES (PMCS): Maintaining equipment in a like new operation condition in accordance with the manufacturers specifications. Accomplished by providing systematic inspection, CVC (Calibration Certification Verific ation), detection, and correction of incipient failures, before they occur or before they develop into a major defect, replacement of cracked or worn buttons, covers, etc. Cleaning and lubrication will be included as needed on all internal parts as well a s external cleaning after services are performed. 3.2. RESPONSE TIME: The time frame, within which the Contractor must arrive on-site, measured from the time of verbal notification that service is required. Response time for this contract is 2 hours after service call is confirmed and deficiency correct ed within 24 hours. 4. CONTRACTOR FURNISHED ITEMS: 4.1. The contractor shall furnish all materials, Original Equipment Manufacturer Parts, labor and supplies necessary to perform the services and repairs. Travel is included in the overall contract cost. 5. SPECIFIC TASKS: 5.1. The Contractor shall provide two scheduled on-site PMCS Services for equipment listed above in #1. Preventive Maintenance calls will be scheduled with the activity in advance. 5.2. Contractors representative shall report to the Biomedical Maintenance / Clinical Engineering Manager Building 9200, Room 039, Martin Army Community Hospital, telephone (706)544-2483, prior to commencing all on-site services and repairs during normal operating hours, 8:30 A.M. to 5:00 P.M., Monday through Friday. If repairs are required after normal duty hours the staff on site will make sure service receipts are provided to Medical Maintenance Branch. 5.3. Upon completion of services, a written service report will be provided to the Biomedical Maintenance / Clinical Engineering Manager. The service report shall be completely filled out, provide detailed information regarding the cause of equipment malf unction and corrective action taken, to include the time required to complete the work, price of labor (hourly rate), and a list of parts replaced with a price for each part. In the event all information is not available to contractors representative whe n services are performed, the initial service report shall include all information stated above except for price. Contractor shall provide the balance of the required information in writing to the Chief, Medical Maintenance, not later than ten (10) days a fter services are completed. 5.4. Upon completion of PMCS, the contractor shall sign off on or affix a DD Form 2163 Calibration verification sticker in accordance with instruction provided in TB 38-750-2. 5.5. The contractor shall perform unlimited emergency calls during normal duty hours, five days per week, excluding legal holidays. Response time for emergency repair will be within two hours after verbal notification is received. Completion time shall b e within 24 hours after arriving on site. 6. SOFTWARE COVERAGE: Customer will receive software upgrades at no additional charge to the government. All upgrades will be installed as soon as the upgrades are available. Any training needed after the upgrades will be the contractors responsibility coordinating with the Department of Pathology. After any software installation or upgrades are completed the operators and service manuals must be updated as well by the contractor.
