- The Carl T. Hayden VAMC, Phoenix, Arizona, is seeking a contractor to provide the services of a substance abuse residential treatment center to eligible beneficiaries. The contractor shall be eligible for JCAHO or CARF accreditation; have a current occupancy permit or license; and be fully compliant with ADA requirements. The contractor?s facility must be located within the Greater Phoenix Metropolitan Area. Contract period shall be for a base year beginning November 1, 2005, or date of award, through September 30, 2006 with 2 additional option years, subject to the availability of funds. This acquisition is unrestricted. North American Industry Classification System (NAICS) 623220 and a size standard of $6 million is applicable to this acquisition. The solicitation shall be made available on or about September 13, 2005 with a due date of October 12, 2005, by 3:00 p.m. local time. Copies of the solicitation may be obtained at no charge at the VA Business Opportunities System website http://www.bos.oamm.va.gov or http://www.fedbizopps.gov/ Dissemination is limited to electronic medium and the VA will provide no hard copies.
