Description
The 347th Contracting Squadron/MSCB is seeking sources to establish a Blanket Purchase Agreement (BPA) for the following: MEDICAL REFERRAL LABORATORY SERVICES 1. DESCRIPTION OF SERVICES: Non personal services to include all labor, tools, machines, and other various items as necessary to provide laboratory services in accordance with medically sound practices to the 347th Medical Group, Moody AFB, GA. 2. PERIOD OF PERFORMANCE: 1 Oct 05 ? 30 Sep 06 with 4 option years. 3. QUALIFICATIONS: Accredition by the College of American Pathologists and be in compliance with Clinical Laboratory Improvement Amendments of 1988. The director should have the appropriate training and background to be able to perform responsibilities as outlined in the College of American Pathologists (CAP) Standards for Laboratory Accreditation for Director and Consulting Pathologist and be present at the laboratory for on-site CAP inspections. 4. WORK HOURS/DAYS: a. Vendor must be able to provide preliminary results of frozen tissue specimens within thirty (30) minutes of receiving specimen b. Vendor must be able to receive emergency blood specimens and frozen tissue specimens transported from Moody AFB within thirty (30) minutes. c. Vendor must be able to process emergency request at all times d. Vendor must be able to transport medical specimens from the 347th Medical Group Laboratory at least two (2) times daily, Monday through Friday, during the hours of 7:30 am to 4:30 pm. 5. GUIDELINES: All tasks shall be accomplished in accordance with all DoD, Air Force, Air Force Special Operations Command, and local base computer/technology guidelines and regulations. The Air Force Instructions (AFIs) will be made available upon request (some are available through on-line reference material). 6. HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY (HIPPA) COMPLIANCE: It is DoD policy that patient sensitive/medical record information be protected in accordance with applicable laws, regulations and guidance as outlined in the Privacy Act of 1974, the Health Insurance Portability and Accountability Act (HIPAA) of 1996, the Freedom of Information Act, Section 756 of the 2001 National Defense Authorization Act, DoDD 5400.11, the DoD Privacy Program and other related guidance. Compliance is mandatory. Please call 229-257-4917 for a copy of the Business Associate Agreement. 7. SPECIAL REQUIREMENT: N/A 8. MATERIALS AND EQUIPMENT: Provide by the contractor. a. Monthly consultation visit to the laboratory to review: I. Quality control documentation for all laboratory-testing procedures in Chemistry, Coagulation, Hematology, Serology, Urinalysis, Microbiology, and Ancillary testing. II. Equipment checks and preventive maintenance documentation for all equipment in Chemistry, Hematology, Serology, Urinalysis, Microbiology, and Ancillary testing. b. Monthly review of all external testing surveys and if applicable, survey exception reports. c. Provide a written report of the evaluation and recommendations for each monthly consultation visit using the Doctors Laboratory, Inc. Laboratory Visit Summary, Form 92-139 or other document providing same documentation. d. Annual review of all operating instructions/procedure manuals and technical advice/recommendations regarding laboratory procedures. e. Calibrate testing equipment to proper guidelines. 9. SPECIFIC TASKS: a. Provide medical referral laboratory services in accordance with medically approved practices. b. Provide consultation to the laboratory staff on abnormal blood smears and to the medical staff regarding medical significance of laboratory findings as appropriate. c. Vendor must provide written test reports, which include patient normal range, call (order) number and other information as required by the College of American Pathologists. d. Vendor must perform whole blood lead testing using the graphite furnace method. The performance period is one year (1 Oct 05 through 30 Sep 06) with four one-year option periods, unless either party cancels the agreement by providing a 30-day written notification.
