Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is FA4830-05-Q-0112 and is issued as a Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-05, Defense Acquisition Circular (DAC) 91-13 Correction and Air Force Acquisition Circular (AFAC) 2005-0818. The solicitation is 100% small business set-aside. The North American Industry Classification System is 335129. The small business size standard is 500 employees. The BID SCHEDULE shall be for CLIN 0001: Re-Lamp Airfield Lighting IAW the Statement of Work; Qty: 1 project. Contractor shall provide price breakout for the following items: Lamps 1000w. HPS ? Estimated 62, Ballasts ? Estimated 19, Obstruction Lamps ? Estimated 21, Labor Hours, Crane & Operator Rental ? Estimated 15 Days DELIVERY / PERFORMANCE PERIOD / LOCATION OF WORK: Moody Air Force Base, Valdosta, GA 31699. All work shall be completed by 15 December 2005. FOB POINT: Destination. Provision at FAR 52.212-1, Instruction to Offerors Commercial Items applies to this acquisition. The following addenda is provided to this provision: Request for Quotation should be submitted and shall contain the following information: SOLICITATION NUMBER; TIME SPECIFIED FOR RECEIPT OF OFFERS; NAME; ADDRESS; TELEPHONE NUMBER OF OFFEROR; TERMS OF THE EXPRESSED WARRANTY; PRICE, ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS. Quote shall also contain all other documentation specified herein. Signed quotes must indicate quantity, unit price and total amount. Offerors that fail to furnish the required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order resulting from this solicitation to the responsive responsible low offeror. STATEMENT OF WORK RE-LAMP AIRFIELD LIGHTING MOODY AFB., GA. 1. LOCATION OF WORK: The location of the work is at Moody AFB,GA. (near Valdosta, GA.) 2. CONTRACTOR?S WORK SCHEDULE: The contractor shall be allowed to work between the hours of 08:00 through 17:00 Monday through Friday. Federal Holidays and weekends are excluded. Work shall be completed by 15 December 2005. 3. SECURITY: The contractor shall obtain a vehicle pass for entry onto the base. The following is required for obtaining a vehicle pass the Visitor?s Center at the South Gate: a. Driver?s License b. Proof of insurance c. Vehicle Registration 4. DESCRIPTION OF WORK: The contractor shall furnish all labor, supervision, materials, and equipment to accomplish the work described below. All work shall be completed by 15 December 2005. The work shall include (but is not limited to) the following: a. In general, the work entails replacement of lamps and ballasts on high mast airfield light fixtures. There are 17 poles with various lighting bank arrangements of 6 to26 lamps. All burned out lamps and ballasts shall be replaced. The poles vary in height from 115 ft. to 150 ft. An 180ft. crane with two-man basket must be used in performing this work. b. The new lamps shall be compatible with the existing fixtures. c. New lamps shall be 1000 watt high-pressure sodium. d. Replace all failed ballasts (assume 30% replacements for all burned out lamps). e. Replace all obstruction lamps that are located on the very top of the pole. There are 2 obstruction lights per pole. Lamps are medium base, 100 w. incandescent. f. Re-aim the light fixtures indicated in Attachment #1 (contact Joseph Schieffer for attachment). g. Replace the missing reflector on T-6/T-38 Pole #7. h. Note: All lights are 277V except for two poles in the T-6/T-38 area at poles # 1 & #2 which are 480 V. These poles can be discerned by their corroded appearance. 5. GENERAL REQUIREMENTS: a. All electrical work shall be done in accordance with the NFPA 70 (latest edition). b. The contractor shall abide by all applicable OSHA safety regulations. c. The contractor shall furnish a limited warranty of 1 year on the lamps and ballasts. The warranty period shall begin on the date of completion and acceptance of work by the Contracting Officer. d. Contractor shall provide limited one year warranty on work and materials. e. Attachment #1 and #2 show the locations of the poles and burned out lamps. This survey was done 29 June 2005. The contractor shall perform a survey prior to commencement of work and identify any additional lamps that have burned out. This shall be reported to the Contracting Officer at Moody AFB.
