Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. A firm fixed-price purchase order is anticipated. Quotations must reference RFQ No. W15QKN-05-Q-0755. The NAICS code is 624190. The Government has a need for Family Advocate Program Services in accordance with the Statement of Work (SOW) included herein. The period of performance will be one year, 25 hours per week, beginning October 2005, with options to extend the period of performance at the sole discretion of the Government up to a maximum of five years. Quotations must include a price for the base year and for each option year. For evaluation purposes, the option prices will be added to the base price. SOW: The contractor shall serve as a Family Advocacy Program Prevention Specialist for the Family Advocacy Program to provide information and instruction on Child Abuse/Neglect and Domestic Violence for the Family Advocacy Services. The contractor shall serve as the Victim Advocate for Family Advocacy and Sexual Assault. In addition, the contractor shall maintain statistical data necessary for the Family Advocacy Program, Sexual Assault Program and the Army Community Service annual reports, as well as coordination of FAP standards and guidelines in preparation for accreditation inspection. 1. The contractor shall provide the following services: a. Conduct training on Domestic Violence and Child Abuse Prevention. b. Provide annual Child Abuse Prevention Training to all Child/Youth Services staff, to include applicants for FCC certification. c. Conduct individual parenting sessions (Family Advocacy Case Review Committee referrals and self-referrals). d. Provide information/referral, support, and advocacy services to Exceptional Family Members??? Program. e. Coordinate the Behavioral Education Assessment Team Services, to include school liaison. f. Serve as the Victim Advocate for Family Advocacy and Sexual Assault Response Programs. 2. The contractor shall conduct specific activities such as Child Abuse Prevention Month (April), and Sexual Assault Awareness Campaigns. The Contractor shall participate in the 2005 Domestic Violence Prevention Month activities (October) and plan and coordinate the 2006 campaign. Coordinate with local media to publicize information on Family Advocacy seminars, workshops arrange for professionals to speak on Family Advocacy Program issues. 3. The contractor shall research, prepare, and write articles monthly on family violence for publication in the newspapers; provide input for contracts to request individuals to conduct specialized training on topics such as parent- child communication, anger management, stress management, family resources and other related issues; maintain community-wide Family Advocacy Program information and referral data to ensure that unit commanders, their staff, and all military personnel and their families are aware of all the information and reading materials offered; prepare information papers and fact sheets as required; maintain statistics on Family Advocacy Program education programs; and assist in developing standards of operation procedures for the Family Advocacy Prevention Program. 4. The contractor shall provide social service programs and referrals for military and civilian community members. 5. Primary worksite: Army Community Services Office, Picatinny Arsenal, New Jersey 07806-5000. The contractor shall work 25 hours per week. Duty hours are flexible, subject to coordination with the Army Community Services Office at Picatinny. Hours may vary due to class schedules and/or Victim Advocate services being needed after duty hours. Required Services and Deliverables: 1. Coordinate, prepare and provide training courses on child abuse and neglect, spouse abuse, sexual assault and methods of prevention. Discuss policy, instructs attendees in recognition skills of child/spouse abuse. Teaches prevention and family wellness skills, emphasizing family strength. Develop pre and post test measurements and lesson plans. Deliverables: Lesson Plans, Attendance Sheets, Class Evaluations, Pre and Post Test and Monthly Statistical Reports. 2. Identify community prevention activity needs through an annual needs assessment, focus groups and questionnaires. Develop contacts with key resource persons. Referral procedures include both emergency and existing assets to extend the amount of support available. Liaison with health and human service agencies that provide peer support and counseling. Deliverables: Needs Assessment, Focus Groups, Questionnaires, MOA/MOU???s and Resource Library. 3. Prepare newspaper and newsletter articles and brochures to educate the community on FAP Programs, Events and Trainings. Employ such communication vehicles as the Voice, Emails, Flyers and ACS Newsletter. Deliverables: Articles, Brochures and Flyers. 4. Attend the Annual FAP Conference for 5 days (location Charlotte, NC), FAST Training for 13 days (location San Antonio, TX) and various trainings recommended by DoD for FAP Staff. Deliverable: After Action Report and Certificate of Training to the ACS Chief. 5. Serve as the Victim Advocate for Family Advocacy and Sexual Assault Response Programs. Attend meetings, train service members on sexual assault prevention, pre-deployment training, response groups training. Input data into database for monthly statistical reports. Deliverables: Meeting minutes, Attendance Sheets, Class Evaluations, Pre and Post tests, Monthly Reports 6. Serves as a member of the ACS Team, being able to assist clients with additional services provided by ACS. Deliverables: Completed Cross Training of ACS programs. Minimum Education/Experience Qualifications: Must possess at least a BS degree from an accredited college or university in the field of Behavioral Sciences, and specialized experience, which has equipped the applicant with the particular knowledge, skills and abilities to perform successfully the duties of the Family Advocacy Prevention Specialist/Victim Advocacy Coordinator position. Experience should involve working with families who have experienced domestic violence, victims of spouse abuse, public speaking, working with groups, and case management. Experience may also include direct social welfare services, application of program policies and operations, collaboration with county, state, and federal agencies/programs, and knowledge of services to military families. Respondents must be registered in Central Contractor Registration in order to be awarded a purchase order in response to this solicitation. The Government plans to issue a purchase order not later than September 27, 2005, to the responsible quoter whose quotation conforms to the solicitation and is determined by the Contracting Officer to represent the best value to the Government; the Government reserves the right to make no award. The following factors will be considered for Best Value, therefore supporting documentation must be included with the price quotation: 1. Knowledge of Domestic Violence, Child/Spouse Abuse Prevention and Sexual Assault Programs and Resources available within the local community and the Army. 2. Ability to develop primary prevention curriculum under AR 608-18 to include pre- and post-measurements, evaluations and lesson plans. 3. Ability to perform training and briefings to Command, active duty military, reserve/national guard, child care providers and DoD civilians on Family Advocacy Prevention Programs (domestic violence, child/spouse abuse prevention and sexual assault).
