Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Womack Army Medical Center Contracting Office intends to ward a contract to Duke Medical Center for Pediatric Cardiologist. Contractor shall provide the nonpersonal services of a physici an to provided pediatric cardiology consultation services. Services shall be provided to all eligible beneficiaries of the military health care system who present themselves to Womack Army Medical Center for treatment. The Contract physician shall be of a qualilty or exceeding current recognized national standards as established by the Joint Commission on Accreditation of Healthcare Organizations (JAHCO). Provider performing under this contract shall remain employees of the Contractor and not the governm ent. Contract is responsible for malpractice insurance for its employees. Womack Army Medical Center will retain full authority and responsibility for the care and treatment of the patients Womack Army Medical Center shall retain administrative control over the contract provider while they are rendering services of patient care nature. Qualifications: Allopathic or osteopthic physicians shall be utilized by the contractor to fullfill contract requirements. Must be licensed to practice in the State of North Carolina. Only residency trained or board certified pediatrician or cardiologist shall render the final interpretation of an electric cardiogram. Must be able to read, write, and speak English effectively in order to effectively communicate with pa tients and WAMC Staff. Hours of Performance: Hours of service shall be between the hours of 0900 - 1500, one or two days per month. Days of service shall be coordinated with the contract 30 days in advance. Specific Tasks: The Contract Cardiologist sha ll perform the following tasks: Provide basic physical exams and histories of both inpatient and outpatient of patients referred to the WAMC Pediatric Department. Provie expert advisory services in the specialty of pediatric cardiology to staff providers at Womack Army Medical Center to include personal views on problem cases within the contract providers scope of specialized training and experience. Condutct diagnostic studies, such as echocardiograms, on patients referred to pediatric cardiology. Exam ine all pediatric patients, newborn through adolescent referred for cardiology medical care to render a medical assessment of their condition and to take appropriate action. Consult with other WAMC staff specialties as appropriate to determine referral pr iorities to patients whose conditions are diagnosed as beyond the capabilities of the healthcare provider or clinic. Prepare referrals thoroughly documenting the patient's condition, history, and reason for referral. Contract physician wil become familia r with WAMC formulary. The contract physician authorized to prescribe pharmaceuticals shall do so accordingly to the availability of drugs listed therein. The pharmacy service will provide instructions to all prescribing practitioners on substitutability of generic drugs for prescribed drugs. Presciptions shall be written on DA1289 and stamped To be filled at WAMC. Contractors are to provide pricing, technical qualifications, and past performance. Pricing is to be broken down into hourly rate and trave l expenses, if applicable. Technical qualifications should address professional qualifications and cabililty of providing required services. Closing date is set for 15 September 2005, 1700 EST. In accordance with 52.212-2, proposals shall be evaluated on price, technical qualifications, and past performance.
