Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The SIC is 7991. The NAICS code is 713940. This solicitation serves as the Request for Quotation for access to Physical Fitness Facility in the Alexandria, Virginia area. Line items are as follows: Item 0001: The Government has a requirement for access to a commercial physical fitness center which is for a twelve (12)month base period and two one year option periods for use by all United States Army Test and Evaluation Command (USATEC) personnel. Thi s purchase order is written to accommodate 142 military personnel, which equates to 710 visits per week. USATEC civilian employees may utilize the fitness facility on a space available basis only. Under no circumstances shall the price to this contract ex ceed payment for greater than 710 visits per week. The contractor will provide a facility and equipment that will be made available to USATEC personnel under the same terms and conditions applied to the general public. Usage may be during the day with pea k times being 6:00am to 3:15pm, excluding weekends and all major holidays. For any personnel entering the Fitness Facility at 3:15pm will have a one-hour grace period. a. No time limitations shall be placed on the use of equipment. b. The contractor shall provide at a minimum the following facilities and equipment: - Indoor track, - Pushup area, - Sit-up benches, - Free weights, - Weight machines and nautilus type equipment, - Lap swimming pool, - And stationary bike area. c. The facility will be located within a ten to fifteen minute driving radius of USATEC Headquarters; preferably within walking distance or provide shuttle service or ensure that parking is available. d. The contractor shall not offer individual membership payment plans to USATEC personnel. Method of employee identification will be the USATEC badge Award will be based on the best value to the Government, which will be determined by evaluating the offerors ability to meet all requirements identified above with consideration given to price and past performance references. To receive an award, the offe ror must be registered in the Central Contractor Registration System and have a valid CAGE Code, tax identification number and DUNS Number. The Government reserves the right to award to other than the lowest price if determined to be in the best interest of the Government. Offers must provide a statement that the proposed facility meets all requirements identified, a brochure of the facility with complete amenities, and the total price that includes a pricing breakout of price per person per day. Award will be made on initial offers. Therefore, offerors must submit their best proposal and pricing, since the Government reserves the right to award on initial offerors without conducting discussions. Provisions at FAR 52.212-1, Instructions to Offers-Commercial and FAR 52.212-4, Contract Terms and Conditions-Commercial Items apply to this acquisition. Offerors need to include a complete copy of the provisions at FAR 52.212-3, Offeror Representations an d Certifications-Commercial Items with their offer (registered in CCR, DUNS Number, Cage Code, and Tax ID Number). The clause at 52.212-5, Contract Terms and Conditions required to implement Statues of Executive Orders-Commercial Items, applies to this acq uisition.
