Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.603 and supplemented with information included in this notice. This announcement is the only solicitation. A written solicitation will not be issued. Quotes are being requested. This requirement, number FA4427-05-Q-0057, is issued as a Request for Quotation (RFQ). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-05 and Defense DCN 20050901. This solicitation/acquisition will be unrestricted with regard to business size standards. The Federal Supply Class (FSC) is Q301. The Standard Industrial Classification (SIC) is 8071. The North American Industry Classification System (NAICS) is 621511. This RFQ consists of the following item. Item 0001 Services non-personal: contractor to provide all personnel, materials, facilities, supplies, and other services and items necessary to perform flow cytometry testing, as well as test result interpretation, when required by the Military Treatment Facility (MTF) during the period 01 Oct 05 through 30 Sep 06. The MTF is located at the David Grant Medical Center, Travis AFB, CA. Projected tests per year are eight (8) rush flow cytometry CPT code 88180; fifty-two (52) rush coagulation studies CPT codes 85420 and 85300; thirty-two (32) complement fixation CPT code 86171; forty (40) immunodiffusion CPT code 86331; eight (8) LS/PG ratio CPT code 84081; and eight (8) Betke-Kleihauer CPT code 85460. The contractor shall comply with the Joint Commission Accreditation Healthcare Organization (JCAHO), Food and Drug Administration (FDA), Privacy Act of 1974, Health Insurance Portability and Accountability Act (HIPAA), and all Federal, State, and local laws applicable to performance of work. The following requirements must be met: (a) flow cytometry testing and interpretation services 365 days per year including weekends and Federal holidays. (b) 24 hour turnaround time from time of specimen receipt for all flow cytometry testing, to include the preliminary interpretation of test results. (c) procedures for flow cytometry testing include but are not limited to leukemia/lymphoma evaluation and HIV staging. (d) contractor shall have a pathologist correlate the specimen morphology with cell surface markers for all leukemia/lymphoma evaluations. (e) provide a pathologist review of the results for all flow cytometry testing. (f) follow NACCLS guidelines for all leukemia/lymphoma evaluations. (g) follow CDC recommendations for all HIV staging. (h) provide a fax copy of the test results as soon as results are available and original test results via regular mail. (i) CAP accreditation and CLIA certification. (j) new medical testing shall not be introduced without prior recommendation and approval of the medical facility?s Commander and the Contracting Officer. (k) incorporate the MTF?s Central Operations patient numbering sequence on reports. (l) provide single-site (one lab) specimen testing for easy specimen tracking. (m) provide advance notification of test changes, additions, and deletions one month in advance. (n) provide the MTF the scheduled days of testing for the different test procedures with turnaround times. (o) provide electronic invoice version on a regular monthly transmission and paper invoices mailed for documentation purposes. (p) provide a client service representative point of contact (POC) that can be reached 24 hours a day, 7 days a week. Furthermore, these 3 service performance objectives must meet 100% compliance: (a) single-site (one lab) specimen testing 365 days a year including weekends and Federal holidays. (b) provide flow cytometry testing within 24 hours of receipt of specimen, to include preliminary diagnosis. (c) properly maintain and dispose of records. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors - Commercial. FAR 52.212-2 Evaluation - Commercial Items, with price as the only evaluation factor. FAR 52.212-3 Offeror Representations and Certifications - Commercial Items. Contractor must ensure annual representations and certifications are current and completed at http://www.bpn.gov/orca/. Contractor must provide a copy of the ORCA printout with the response to this RFQ. The Government?s obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.
