Description
USDA Wisconsin Farm Service Agency (FSA) requires the services of qualified persons or firms to provide FSA Farm Loan Programs the following services beginning January 3, 2006: January 1, 2006 balance sheet information from specified FSA borrowers. 2005 actual income, expense and production data on specific farm borrowers. Income, expense and production projections for specified farm borrowers. All work will be completed electronically as well as in paper copy with original signatures. Two methods of electronic submission will be acceptable. a) Data file utilizing Web Equity Manager (Web Equity Manager is a registered trademark of ECI) b) Electronic spreadsheet ? format file provided by FSA. Workbook #1 is Form FSA-2037 and Workbook #2 is Actual Income and Expense Summary. 1. Task 001 ?Farm Business Plan Worksheet (Balance Sheet),? Form FSA-2037 and Balance Sheet Schedules Input a) The contractor will meet with the FSA farm borrower and complete Form FSA-2037, ?Farm Business Plan Worksheet (Balance Sheet)? and Balance Sheet Schedules Input. This form will be completed showing information as of January 1, 2006 and will be completed in detail (pages 1 ? 4) based upon a meeting with the borrower. b) The borrower and co-applicant?s signature and date will be obtained on the paper copy. c) The contractor will provide a short summary of the meeting with the farm borrower detailing the date they met with that borrower and who the contractor?s representative was who obtained the information. 2. Task 002 (Actual Income and Expense Summary Input and the related schedules) 2005 : a) The contractor will meet with the FSA farm borrower and complete Exhibit B by reviewing the monthly financial and production record entries used to generate the annual summary of the family's financial record-keeping system . b) The contractor will obtain the borrower and co-applicant?s signatures on page 2 of the Summary Input of Exhibit B. c) The contractor will attach a summary of the meeting with the borrower(s) which will include as a minimum the date of the meeting(s), the name of the contractor?s representative who met with the borrowers, the persons the contractor met with and any facts on production, payments made, decrease or increase in debts, borrower needs and observations made by the contractor that should be considered in any future loan making assessments including but not limited to: an assessment if the existing record keeping system is up to date and accurate. 3. Task 003 Projections (2006 Projection Income and Expense Summary Input and the related schedules: a) The contractor will meet with the FSA farm borrower and complete Exhibit C by reviewing the last 3 years historical information, if available, taking into consideration of any future changes. Income projections will be based on the 2006 PLANNING PRICE GUIDELINES. b) The contractor will obtain the borrower and co-applicant?s signatures on page 2 of the Summary Input of Exhibit C. c) The contractor will attach a summary of the meeting with the borrower(s) which will include as a minimum the date of the meeting(s), the name of the contractor?s representative who met with the borrowers, the persons the contractor met with and any discussions on deviations of historical yields and production, and the established planning prices used. In addition the summary will include any other information that FSA should be made aware of.
