Description
This is a combined synopsis and solicitation for the following commercial items, The repair and/or upgrade/modernization of three elevators in the 30th Medical Group Hospital The 30th Medical Group has experienced various problems with the elevators working properly. The Elevators have frequently shutdown, locked out or become completely inoperable. 30th Medical Group requests a site proposal be conducted to determine best upgrade/modernization and repair of three elevators in this four story building located at 338 South Dakota Ave, Vandenberg AFB, CA. The Contractor shall provide all management, transportation, tools, supplies, equipment, and labor necessary to upgrade/modernize and repair the elevators at the 30th Medical Group in accordance with all local, state, federal laws in a manner that will ensure continuous and safe operation The contractor shall perform, required upgrades and repairs of elevators, in accordance with manufacturer specifications, and National Elevator Maintenance. The contractor shall comply with applicable American Society of Mechanical Engineers/American National Standards Institute (ASME/ANSI) Codes. All repair work shall be in accordance with normal commercial practices using parts specified by the elevator manufacturer or items of equal or better quality. Downtime of the elevator will be kept to an absolute minimum. The contractor shall schedule the repairs with the 30 MDG facilities Manager. Contractor shall warranty all parts and labor for one year from installation completion date. The contractor shall prepare and submit a report of all completed work within five (5) business days after completion of work. The report shall identify repair work performed, date of repair, warranty information, name of inspector and/or repair technician, overall condition of the elevator, and actual time spent at the work site. Elevator maintenance and repair services must conform to all applicable standards and codes. These include, but are not limited to: ANSI/ASME A17 ? Safety Code for Elevators, Dumbwaiters, Escalators, and Moving Walks U.S.C. Title 42, 126, III ? Americans With Disabilities Act of 1990 State Codes such as: California Elevator Safety Construction Code (Title 24, Part 7, 1995) Building and Equipment Standards and Specifications such as: ASME QEI-1-1993 ? Qualification of Elevator Inspectors and ISO 4101:1983 ? Drawn Steel Wire for Elevator Ropes OSHA Standards (29 CFR) 1917.116 ? Elevators and Escalators Local and State Building Codes Upon completing the upgrade/repair, a state certified inspection needs to be performed (at the cost of the contractor) and all certification met. Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements in maintenance of elevators. Contractor shall submit copies of the certification(s) to the CO within 10 days after contract award. No proprietary equipment shall be installed and existing proprietary equipment shall be replaced. There will be a site visit at the Vandenberg Hospital, building 13850 Thursday September 1, 2005 at 10:00 AM local time if you are interested in attending please provide me the name of the individual or individuals who will be attending no later than 4:00 pm local time Tuesday August 30, 2005. This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ) FA4610-05-T-0031. Submit written offers, oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). This solicitation document incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05 and DFARS Change Notice 20050726. North American Industrial Classification Standard 333921, Size Standard, 500 employs, applies to this procurement. FOB: Destination for delivery to Vandenberg AFB 30 MDG building 13850, Vandenberg AFB CA 93437 .The following provisions and clauses apply to this acquisition: FAR 52.211-6 Brand Name or Equal. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items. Quotes will be evaluated on price related factors only. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, and the offeror must include a completed copy of this provision with their proposal. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items. Question and Answers 1. Do you need to upgrade the buttons to make the elevators ADA compliance? No there buttons are already in compliance with ADA standards? 2. Will there be any cosmetic changes to the appearance to the elevators? No, the only thing that needs to be upgraded is the operation of the elevators the elevators are fairly new and therefore we do not need upgrade 3. The current motors are DC motors as part of the upgrade do you want these three transformer and motors changed to more efficient AC motor. Yes we would like these motors replaced 4. Currently you have a contract to have the current elevators maintained how will this work with the upgrade? Contractor would be responsible for maintain everything under their current contract. Upgrade contractor would be responsible to maintain anything for the warranty period for upgrades. 5. Please identify the manufacturer and model number for each elevator. Normic 1600 6. When was each elevator originally put into service? 1996 7. The scope of work indicated that the work is to upgrade/modernize and repair the elevators in accordance with all local, state, federal laws. Does this include providing seismic upgrades to the elevators, including structural retrofit of the elevator shafts and supporting structural framework? The intent to only perform equipment upgrades/modernizations and repairs for increased operational reliability and to meet fire and electrical safety codes? 8. Are any bid, performance and/or payment bonds required for this contract? No performance or payment bonds will be required
