Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12, in conjunction with FAR 13.5, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 4THO21057263 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 05-04. A. Scope of Contract: The purpose of this Request for Proposal is to acquire hardware as requested by the customer for Solicitation Number 4THO21057263. The contractor shall perform the following tasks: 1. Acquire the hardware as indicated on the previous page of this addendum to the SF 1449 2. Delivery Requirements: 30 Days ARO Instructions to Offerors-Commercial (JAN 2005) The offeror must complete the annual representations and certifications electronically via the ORCA website at http://orca.bpn.gov. After reviewing the ORCA database information, the offeror verifies by submission of this offer that the representations and certifications currently posted electronically at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including business size standard applicable to the NAICS code referenced for this solicitation), as of the date of this offer and are incorporated in this offer by reference (see FAR 4.1201); FAR 52.212-4, Contract Terms and Conditions ? Commercial Items, applies to this acquisition. 52.212-2 Evaluation-Commercial Items As prescribed in 12.301 (c), the Contracting Officer may insert a provision substantially as follows: EVALUATION?COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. The offeror?s proposal will be checked to ensure it contains the technical acknowledgement indicating the understanding and willingness to comply with the Terms & Conditions listed in the solicitation. If the offeror submits any exceptions, assumptions or conditions by which the offer was made, they will be evaluated to ensure the government obtains what it is contracting for. 2. The offeror?s past performance references will be checked and must found to be relevant and satisfactory in order to receive an acceptable rating. The offeror must submit a completed Contract Performance Questionnaire for 2 similar projects performed within the past eighteen months. Offeror?s are required to use Attachment A for the questionnaire. 3. The Offer?s price proposal will be evaluated for reasonableness. Technical and past performance, when combined, are __________ [Contracting Officer state, in accordance with FAR 15.304, the relative importance of all other evaluation factors, when combined, when compared to price.] (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer?s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
