- COMBINED SYNOPSIS Asbestos Surveys for Malmstrom AFB (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ) for implementation of a Blanket Purchase Agreement (BPA). Submit written quotes (oral quotes will not be accepted) on RFQ reference number 341CEV-1. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-04. (iv) The North American Industry Classification System (NAICS) code is 562910. (v) This combined synopsis and solicitation is for the purchase of asbestos survey services for Malmstrom AFB, MT. (vi) 1. INTRODUCTION This statement of work (SOW) defines requirements for asbestos containing materials (ACM) surveys and related documentation. The place of performance shall be at selected buildings on Malmstrom AFB. 1.1 SCOPE Assessments under this scope include identification, sampling and analysis of suspect ACM from both the interior and exterior of facilities. ACM survey results will specify the quantity of ACM in either linear or square feet; the characteristics, condition, and the location of the ACM within the building; provide marked drawings of the building floor plan indicating the location of ACM; and specify the condition of existing ACM using the AHERA system. The Contractor shall generate computer-aided design (CAD) drawings from blueprints and existing CAD drawings for documenting the findings of the surveys. The Contractor shall: furnish the personnel, services, equipment, materials, facilities, and other requirements necessary for, or incidental to, the performance of work set forth herein. Individuals that are accredited by the Montana Department of Environmental Quality shall perform technical services. 2. APPLICABLE DOCUMENTS The Contractor shall comply with all applicable (1) federal, state, and local environmental statutes, instructions, manuals, handbooks, regulations, guidance, policy letters, and rules (including all changes and amendments), and (2) Presidential Executive Orders, in effect on the date of start of performance. 3. ADMINISTRATIVE AND MANAGERIAL REQUIREMENTS The Contractor shall perform management and planning functions, as well as performance measurement and cost status reporting, during the course of this effort. 3.1 MEETINGS AND CONFERENCES 3.1.1 Post-Award Meeting The Contractor shall plan, schedule, and participate in a kick-off meeting immediately following receipt of the notice to proceed. The Contractor shall prepare an agenda for this kick-off meeting, including a schedule for work completion, a list of individuals assigned to the project and their responsibilities, and resources required and how they will be obtained. The project schedule and milestones for delivery of draft and final documents will be discussed at the kick-off meeting and the Contractor shall submit these meeting minutes as a deliverable, including the proposed schedule with milestones for deliverables. 3.1.2 Progress Meetings. The Contractor shall conduct site visits as necessary to accomplish requirements of this SOW and to discuss work progress with base personnel. The final site visit shall include a meeting with base operations and maintenance personnel, the Contractor, and the base point of contact (POC) to discuss the survey findings, to include a visit to each facility inspected to identify specific findings. 3.2 WORK SITE REQUIREMENTS 3.2.1 Safety The Contractor shall be responsible for protecting the lives and health of employees and other persons, preventing damage to property, materials, supplies, and equipment, avoiding work interruptions and complying with Occupational Safety and Health Administration (OSHA) safety and health regulations and the base safety office requirements. All on-site workers (contractor and subcontractor) performing hazardous operations, including working with hazardous materials, must have completed the OSHA 1910.120 Hazardous Waste Operations and Emergency Response (HAZWOPER) training and/or other applicable training, plus annual refresher courses. The Contractor shall maintain documentation supporting training records and have a written Health and Safety Plan on-site available for workers and/or regulatory review. The Contractor shall provide the CO copies of any OSHA report(s) submitted during the duration of this agreement. 4. SERVICES 4.1 REPORTING AND DOCUMENTATION The Contractor shall provide the base with a survey report for each facility inspected. The report shall include, but not be limited to, the following. 4.1.1 An executive summary that provides a description of the building and its construction, descriptions of floor and wall coverings and surface finishes, homogeneous areas that were identified, inspection dates, and inspectors’ names. The executive summary shall provide a summary of suspect ACM identified and sampled, ACM identified and its locations and possible ACM that was inaccessible for inspection. The Contractor shall point out any damaged or friable ACM requiring immediate action to protect personnel from exposure. 4.1.2 Survey data shall be provided in tables and on drawings or figures as appropriate, and shall include, but not be limited to, the list below. Type material and condition of all suspect ACM and LBP Homogenous areas Building number Sample number Sample location and ACM locations in three dimensional coordinates within sampled spaces Detailed description of material Analytical results AHERA ranking factors Quantity of material observed CAD drawings showing all sample locations and detailing locations of ACM Locations of possible suspect ACM that was inaccessible for inspection (inside pipe chases, walls, etc.) 4.1.2.1 Sample numbers, analytical results, material descriptions, location descriptions, references to photograph numbers, and estimated quantity of ACM, shown on one table for ease of use in the field. 4.1.2.2 Color photographs of sampling locations and materials identified by analysis to be ACM or presumed to be ACM (PACM) shall be numbered and included with detailed captions and arrows or other marks on the photos to point out sampling locations. 4.1.3 The Contractor shall quote and make interpretations of EPA, MDEQ and OSHA requirements for abatement of identified ACM or for demolition of structures containing the identified ACM. 4.1.4 Documentation of qualifications of inspectors including copies of MDEQ accreditation certificates for asbestos inspectors shall be included as an appendix. 4.1.5 Copies of all laboratory analytical reports shall be included as an appendix. 4.1.6 The contractor shall furnish the Government one electronic copy of the report, including all appendices, on floppy or compact disks. The contractor shall furnish two paper copies of the report, to include 8 ½ x 11 and 11 x 17 black/white and color as required. All report pages shall be printed double-sided wherever possible. Foldout drawings or maps can be printed single-sided. 4.2 ASBESTOS SURVEY 4.2.1 The Contractor shall perform technical activities and analyses in support of requirements to comply with federal, state, and local rules and regulations regarding asbestos. The Contractor shall conduct activities necessary to identify the presence of asbestos containing materials (ACM) in facilities and equipment. These activities include performing a thorough review of government-provided building records to include drawings, available specifications, past removal actions, inspection reports, and reports/findings of independent asbestos surveysand collect bulk samples of materials suspected of containing asbestos and submit for laboratory analysis. 4.2.2 The Contractor shall develop controls and protocols to minimize occupational exposure to asbestos during the survey. 4.2.3 The Contractor shall have all representative bulk samples analyzed by polarized light microscopy (PLM) and Transmission Electron Microscopy (TEM) in accordance with current EPA methods. Only laboratories accredited under the National Voluntary Laboratory Accreditation Program (NVLAP) and approved by the state of Montana shall conduct the bulk sample analyses. 4.2.4 The Contractor shall label, with warning labels conforming to OSHA Regulation 29 CFR 1910.1001, all friable ACM found on mechanical systems and insulated water lines. 5. METHOD OF PAYMENT Payments will be made through electronic means – Government Purchase Card – when service is provided. 6. PERFORMANCE LINE ITEMS Requirements for each call-out are as follows: 6.1 ITEM NO 0001: MOBILIZATION (EA) Shall consist of all labor, vehicle mileage, lodging, and per diem necessary to travel to and from Malmstrom AFB to conduct asbestos surveys. The number of mobilizations required will be negotiated with each call and will be dependent on the number of facilities to be surveyed. 6.2 ITEM NO 0002: ASBESTOS INSPECTOR (HR) Shall consist of all work necessary to conduct asbestos surveys of identified facilities. Asbestos inspectors, accredited by the MDEQ, shall conduct surveys and inspect specified areas to identify locations of all suspected ACM and determine if material is friable. Inspectors shall submit copies of their current MDEQ asbestos inspector accreditation to the base point of contact before beginning work. The hourly rate for Asbestos Inspector shall include furnishing all labor, tools, supplies, equipment and incidentals necessary to complete the surveys. 6.3 ITEM NO. 0003: BULK SAMPLE ANALYSES–PLM (EA) Shall consist of analyzing suspect ACM by PLM. The unit price for each type of sample analyses shall include all packing, shipping and analytical costs for each sample. 6.4 ITEM NO. 0004: BULK SAMPLE ANALYSES–TEM (EA) Shall consist of analyzing suspect ACM by TEM. The unit price for each type of sample analyses shall include all packing, shipping and analytical costs for each sample. 6.5 ITEM NO. 0005: REPORT PREP–PROFESSIONAL/NON-PROFESSIONAL (HR) Shall consist of all work necessary to prepare the inspection report as specified in section 4.1. The hourly rate shall include all professional/non-professional labor, equipment and incidentals necessary to prepare the required documentation. Providing required floppy discs or compact disks of the report shall be considered incidental to report preparation.
