Description

Request For Proposal Service Agreement For Water Treatment/Purification Hemodialysis Clinic RFP-598-111-06 Requests for Quotations are currently being accepted for service agreement to provide all labor , materials and transportation to provide water of adequate purity for the hemodialysis clinic at John L McClellan Veterans Memorial Hospital, 4300 West 7th Street, Little Rock, Arkansas for the period of October 1, 2005 through September 30, 2006. This proposed contract action is unrestricted. It is our intent to award as a sole source to U. S. Filter but all responsible parties may submit a proposal. DESCRIPTION/SPECIFICATION/WORK STATEMENT Requirements and Specifications: Contractor shall provide all services necessary to provide service which will assure water of adequate purity and quantity for the operating order of the Hemodialysis Clinic, Ward 6B, in accordance with standards set forth b the American Dialysis Association for John L. McClellan Memorial Veterans Hospital, Little Rock, Arkansas. Contractor will provide services in accordance with all the terms, conditions, provisions and specifications (but not limited to the following to assure the adequate purity) for the period of October 1, 2006 through September 30,2006 with two (2) one year options to be exercised at the discretion of the government.. These requirements and specifications are to include (on site when possible). 1. At a frequency of at least once per week, or as needed change prefilters, check softener, add salt as necessary and monitor reverse Osmosis System. 2. Clean or Replace six (6) membranes in the reverse Osmosis System annually. 3. At a frequency of at least once every three (3) months, exchange one 3.6 carbon tank. 4. Total-disinfect ? Contractor to coordinate with Biomedical Engineering. Provide 24 hr notice prior to day to disinfect. Preferred days: Tuesdays and Thursdays after 3:00 P.M., this at a frequency of every three (3) months. 5. At a frequency of at least once every three (3) months, or as needed, exchange one (1) carbon tank, two (2) mixed bed deionizers. 6. At a frequency of at least once every three months, change out 0.2 filter #FCWSF0952. 7. Contractor will sign in and out through the Plumbing Shop, Room GC114F, John L. McClellan Memorial Veterans Hospital, Little Rock Division and upon completion of service shall have the Supervisor, Plumbing Shop, or the Contracting Officer?s Technical Representative sign a service report verifying work accomplished. A copy of the signed service report will be provided to the Supervisor, Plumbing Shop or the Contracting Officer?s Technical Representative. 8. Contractor will contact the Supervisor, Plumbing Shop or Program Assistant, Telephone Number (501)-257-5429, to schedule maintenance inspections. Work shall be performed from 8:00 A.M. through 4:30 P.M. Monday through Friday excluding Federal Holidays, except in emergency situations as identified by a Hospital Representative. 9. Failure to provide documentation for record as required or failure to furnish service report as required may result in non-payment of charges. 10. The government reserves the right to have a Plumbing Shop technician present during contract maintenance. 11. The contractor must be in possession of all specialized test equipment needed to provide service in accordance with American Dialysis Association. All responsible sources may submit a quotation.
